Job PurposeTo provide high-quality nursing care to patients registered with the GP practice, working as part of the multidisciplinary primary care team. The Practice Nurse will assess patients, deliver treatment and preventative healthcare, support long-term condition management, and promote health and wellbeing in accordance with NHS standards and professional requirements.

Key Responsibilities

• Conduct patient assessments, including clinical observations and nursing reviews.
• Provide wound care, dressings, suture and clip removal where appropriately trained.
• Administer vaccinations and injections in accordance with national guidance.
• Carry out NHS health checks and health promotion activities.
• Support the management and monitoring of long-term conditions such as:
• Diabetes
• Asthma and COPD
• Hypertension
• Cardiovascular disease
• Chronic kidney disease
• Undertake blood pressure, pulse, temperature, oxygen saturation and other appropriate clinical measurements.
• Perform ECGs and other investigations within competence.
• Take blood samples and other specimens where trained.
• Provide cervical screening where appropriately qualified.
• Provide contraception and sexual health support within scope of practice and training.
• Give patients advice on lifestyle, smoking cessation, diet, exercise and disease prevention.
• Recognise deteriorating or acutely unwell patients and take appropriate action, including escalation to a GP or emergency services.
• Maintain accurate, confidential and contemporaneous patient records.
• Work collaboratively with GPs, healthcare assistants, pharmacists, community nurses and other members of the multidisciplinary team.
• Participate in clinical audits, quality improvement and practice development.
• Maintain infection prevention and control standards.
• Ensure safe handling, storage and disposal of medicines, vaccines and clinical waste.
• Maintain professional knowledge and competence through continuing professional development.

Professional RequirementsThe post holder should:

• Be a Registered Nurse (RN) with the NMC.
• Have current NMC registration and comply with the NMC Code.
• Have experience in primary care, community nursing or a relevant clinical setting, depending on the role.
• Demonstrate good clinical assessment and communication skills.
• Be able to work independently while recognising when to seek advice or escalate concerns.
• Maintain patient confidentiality and comply with relevant information-governance requirements.
• Demonstrate commitment to safeguarding adults and children.
• Participate in mandatory training and continuing professional development.
