Job summary

Working as part of the Neighbourhood Mental Health Team, the post holder will provide a range of interventions to assess and support the needs of people registered with a GP in the borough.

Provide leadership to a range of Pennine Care and voluntary, community, social and Faith-based Enterprises (VCSFE) colleagues and will be aligned with our existing Integrated Neighbourhood teams and Primary Care Networks, to ensure that referrals received in the Neighbourhood Mental Health Team, are effectively reviewed and signposted.

To facilitate daily team huddles and contribute to the development and operation of the weekly Neighbourhood MDT meetings.

Main duties of the job

To support the management process of referrals into the Neighbourhood Mental Health service

To provide effective triage within the MDT huddle and provide oversight of formulations and agreed plans/outcome

To contribute to signposting to alternative services, or escalation to crisis services where this is clinically indicated

To facilitate daily huddles, and provide leadership for complex cases and care planning

Be responsible for identifying specific skills and interventions required from the MDT to deliver care and support recovery

To complete biopsychosocial mental health assessments and risk assessments to understand a person's care and support needs and develop care/safety plans

Be responsible for formulation, implementation, and evaluation of care by

To ensure that individual plans are developed in agreement with the person and agreed by the wider MDT

To provide evidence-based interventions for people presenting with care and support needs because of their mental health

To ensure effective interface between health, social care and VCSE partners

To work closely with specialist community mental health teams to facilitate an easy in, easy out approach ensuring that people are able to access the services they require within a timely manner

Provide consultation, triage, advice and liaison within the Integrated Neighbourhood Teams

Develop and maintain effective lines of communication within Health, Social Care and VCSE and ensure that individual needs, care plans and outcomes are shared

Support people to manage their physical health by working in collaboration with GPs and Integrated Neighbourhood Teams
