Main duties of the job

Main Duties and ResponsibilitiesThe successful candidate will be expected to independently manage and review patients across a range of long-term condition disease registers, including:

  • Diabetes
  • Asthma
  • COPD
  • Hypertension
  • Coronary Heart Disease
  • Chronic Kidney Disease
  • Stroke/TIA
  • Heart Failure
  • Learning Disabilities reviews
  • Other long-term conditions as appropriate

Additional responsibilities will include:

  • Conducting annual reviews and monitoring of patients with chronic diseases.
  • Developing and implementing personalised care plans.
  • Undertaking clinical assessments and examinations.
  • Prescribing medications within competence and scope of practice.
  • Reviewing and adjusting treatment plans in line with national and local guidelines.
  • Delivering health promotion and preventative healthcare interventions.
  • Vaccinations and immunisations.
  • Cervical cytology
  • Supporting Quality and Outcomes Framework (QOF) achievement as well as local and national incentives.
  • Maintaining accurate clinical records and coding.
  • Participating in practice clinical audits and service improvement initiatives.

  • Qualified and competent in the fitting, removal, and management of Long-Acting Reversible Contraception (LARC), including contraceptive implants (Nexplanon/Implanon) and intrauterine contraception (coils).
  • Evidence of relevant accredited training and maintenance of competency in LARC procedures.
  • Conducting annual reviews and monitoring of patients with chronic diseases.
  • Developing and implementing personalised care plans.
  • Undertaking clinical assessments and examinations.
  • Prescribing medications within competence and scope of practice.
  • Reviewing and adjusting treatment plans in line with national and local guidelines.
  • Delivering health promotion and preventative healthcare interventions.
  • Vaccinations and immunisations.
  • Cervical cytology.
  • Supporting Quality and Outcomes Framework (QOF) achievement as well as local and national incentives.
  • Maintaining accurate clinical records and coding.
  • Participating in practice clinical audits and service improvement initiatives.

Essential Criteria

  • Registered Nurse (NMC registered).
  • Independent Nurse Prescriber qualification.
  • Experience working within General Practice or Primary Care.
  • Proven experience managing long-term condition clinics and disease registers.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Proficient in clinical IT systems and electronic patient records.

Desirable Criteria

  • Chronic Disease Management qualifications.
  • Cervical Cytology qualification.
  • Spirometry qualification.
  • Diabetes diploma or equivalent.
  • Experience of EMIS Web.
  • Mentor or teaching qualification.
