Job summary
An exciting opportunity within Practice Plus IUC as an Advanced Clinical Practitioner within the new Clinical Resilience Team, providing enhanced clinical assessment and decision-making for patients accessing urgent care services. You will use telephone, video and online consultations to assess, diagnose and agree safe onward management plans, ensuring patients receive the most appropriate care without unnecessary repeat assessment. Working within the Clinical Assessment Service and alongside NHS 111 and Out of Hours teams, you will play a vital role in delivering high-quality, evidence-based care 24 hours a day, seven days a week. You will work autonomously within your scope of practice while contributing to a collaborative multidisciplinary environment. This is an opportunity to make a real difference to patient outcomes by providing timely clinical advice, prescribing where appropriate and supporting colleagues across urgent care services. Your expertise will help maintain safe, effective and responsive services for patients of all ages. You will also support service quality, clinical governance and the development of innovative models of care, helping Practice Plus IUC deliver outstanding patient-centred services. This is a fully remote role with an opportunity to support every part of our service as a specialist clinician to assist with the changing demand across the UK. Main duties of the job
Deliver advanced clinical assessment, diagnosis, advice and treatment planning through telephone, video and online consultations. Manage patients requiring enhanced clinical assessment and make timely decisions regarding referral, treatment, self-care or discharge. Prescribe safely within your scope of practice and agreed prescribing frameworks. Assess a wide range of adult and paediatric presentations, including complex and undifferentiated cases. Provide clinical support and specialist advice to multidisciplinary colleagues across NHS 111, Out of Hours and Clinical Assessment Service teams. Support validation of urgent dispositions and direct patients to the most appropriate services for their needs. Manage prescription requests and contribute to efficient patient care pathways using electronic prescribing systems. Maintain accurate clinical records, follow policies and protocols, and actively contribute to clinical governance activities. Participate in training, supervision, appraisal and continuous professional development activities.

About us
Current NMC or HCPC registration and MSc in Advanced Clinical Practice or equivalent qualification V300 (or equivalent) non-medical prescribing qualification. Extensive experience of telephone-based clinical assessment and triage. Strong primary care experience, including consultation, triage and holistic patient assessment. Confidence assessing, treating and safely managing patients of all ages, including young children. Excellent communication, listening and record-keeping skills with a patient-focused approach. Ability to work autonomously, manage pressure effectively and collaborate as part of a multidisciplinary team.
