Job summary

Cranleigh Medical Practice is expanding its Urgent On-the-Day Care Team and is looking for an experienced and motivated Registered Nurse or Paramedic to join us in an Advanced Clinical Practitioner (ACP) development role.

This is an exciting opportunity for an experienced clinician who has developed advanced assessment skills and is keen to build on their existing experience and progress their career within primary care.

Main duties of the job

We are looking for a clinician who:

• Is a registered Nurse or Paramedic with a minimum of five years experience in primary care, the ambulance service, or an acute care setting.
• Has completed a postgraduate qualification in advanced clinical assessment (or equivalent) and can demonstrate experience in advanced assessment and clinical decision-making.
• Is committed to developing their skills and scope of practice within urgent primary care.
• Is willing to undertake a non-medical prescribing qualification as part of their development, if not already qualified to prescribe.

Job description

The Advanced Clinical Practitioner -ACP- is an experienced clinician working autonomously within the Cranleigh Medical Practice clinical team. The postholder will have a Level six or seven clinical diagnostic and assessment qualification. You will have a minimum of five years experience in urgent care, primary care, or an acute clinical setting, delivering safe, effective, and person-centred care across a broad spectrum of presentations both acute and chronic for adults and children -aged 1 year and above. As this is a development role, training will be given to a suitable candidate in relation to paediatrics.

Working both independently and as part of a multidisciplinary team, you will assess, diagnose, treat, and manage patients while upholding the highest professional standards. The role involves telephone and face-to-face consultations, supported by clinical governance, mentoring, and supervision from GPs and senior ACPs.

Key Responsibilities: Clinical Practice Undertake comprehensive clinical assessments for patients face-to-face, telephone, or home visits.

Perform holistic examinations considering physiological, psychological, and social factors.

Formulate and implement evidence-based management and treatment plans for both acute and long-term conditions.

Prescribe and review medications safely in line with national and local guidelines, ensuring therapeutic effectiveness and patient understanding once prescribing qualification is completed.

Manage risk and prioritise care effectively in urgent or emergency situations, initiating appropriate interventions.

Promote health and wellbeing by supporting patients in adopting self-care and preventative strategies.

Communication Demonstrate excellent communication and consultation skills to ensure shared decision-making and informed consent.

Act as an advocate for patients, ensuring their needs and preferences are respected.

Communicate effectively with patients, carers, colleagues, and external healthcare professionals, using appropriate methods and adapting to overcome communication barriers.

Maintain clear, professional, and accurate records in line with legal and organisational standards.

Quality and Governance Deliver care within professional standards and the regulatory frameworks of the NMC or HCPC.

Adhere to NICE guidance and evidence-based best practice.

Participate in clinical audit, quality improvement initiatives, and peer review to continuously enhance care delivery.

Contribute to the practices clinical governance and risk management processes, including incident review, root cause analysis, and shared learning.

Identify opportunities for service improvement and contribute to the development and implementation of local protocols and pathways.

Maintain up-to-date safeguarding knowledge for both adults and children.

Leadership, Education, and Development Take responsibility for your own professional development through reflection, clinical supervision, and continuing professional education.

To lead basic life support training sessions for clinical and administrative staff.

Act as a role model and resource for colleagues, promoting excellence and professional integrity.

Support the development and education of junior staff and students through mentoring and clinical supervision.

Contribute to service innovation and leadership within the multidisciplinary team.

Participate in teaching, case discussions, and multidisciplinary meetings to support shared learning.

Teamworking Work collaboratively with GPs, nurses, paramedics, and administrative teams to deliver coordinated and high-quality patient care.

Contribute to developing new models of care and improving efficiency within the practice.

Delegate appropriately and support colleagues in their professional roles, ensuring safe and effective care delivery.

Participate in team meetings, clinical case reviews, and planning discussions to optimise patient outcomes.

Risk Management and Health & Safety Identify, assess, and manage clinical and environmental risks, ensuring a safe working environment for staff and patients.

Adhere to infection prevention and control standards and contribute to maintaining a clean and safe clinical environment.

Follow all health and safety policies, reporting hazards or incidents promptly.

Participate in mandatory training and ensure personal security systems are used in accordance with practice policy.

Confidentiality and Information Governance Maintain strict confidentiality in all matters relating to patients, colleagues, and the organisation.

Handle all information in compliance with Data Protection legislation and practice policies.

Use IT systems responsibly and accurately for patient care, communication, and data management.

Be aware of obligations under the Freedom of Information Act 2000.

Supervision and Support

The postholder will be supported by GPs through regular supervision, mentorship, and case review. The level of supervision will be proportionate to the ACPs experience, competence, and confidence, as determined by clinical governance arrangements.
