We are seeking a flexible salaried GP to join our friendly PCN team, with opportunities for remote, face-to-face, or hybrid working. This role is ideal for someone balancing childcare or other commitments, offering the flexibility to work from home while remaining fully supported by an experienced practice team.

You can choose to attend practices for teaching, learning, collaboration, and team support as much or as little as you wish.

We are flexible on days, hours, and working patterns, helping you create a role that works for you while continuing to deliver excellent patient care.

Main duties of the job

Clinical Responsibilities

The postholder will:

• Deliver high-quality, patient-centred clinical care.
• Undertake face-to-face consultations across PCN sites.
• Conduct telephone and video consultations.
• Assess, diagnose and manage acute and chronic medical conditions.
• Initiate, interpret and act upon investigations appropriately.
• Develop evidence-based management plans.
• Prescribe safely and effectively in line with local and national guidance.
• Review pathology results and clinical correspondence.
• Refer patients appropriately to secondary care and community services.
• Participate in proactive management of patients with long-term conditions.
• Undertake home visits where clinically indicated.
• Contribute to the delivery of enhanced services and population health initiatives.

MAIN DUTIES AND RESPONSIBILITIES

Hybrid and Remote Working Responsibilities

The postholder will:

• Undertake designated remote consulting sessions.
• Complete clinical administration and correspondence remotely where appropriate.
• Maintain high standards of confidentiality and information governance.
• Communicate effectively across all practices and teams.
• Utilise approved NHS digital platforms and clinical systems.

Team Working and Collaboration

The postholder will:

• Work collaboratively with colleagues across the PCN.
• Attend clinical meetings, educational events and MDT discussions.
• Support the development of integrated patient pathways.
• Work alongside ARRS and community teams to improve patient care.
• Contribute to service innovation and quality improvement programmes.
• Foster positive and professional working relationships.

• Enhanced Care Visiting Paramedic supporting frail and housebound patients.
• Nursing Associates providing clinic-based and domiciliary care for patients with long-term conditions.
• Clinical Nurse Lead supporting the practices with supervision, mentorship and compliance.
• First Contact Physiotherapy Practitioners delivering direct access musculoskeletal services.
• Clinical Pharmacists, Pharmacy Technicians and Care Coordinators supporting medicines optimisation and chronic disease management.
• Social Prescriber helping patients access community resources and non-clinical interventions.
