Job summary

As an Advanced Clinical Practitioner for Keynsham Primary Care Network, you will be a registered Nurse or Paramedic, providing expert autonomous clinical care to patients across the full range of presenting conditions. Working at an advanced level of practice, the ACP undertakes comprehensive assessment, diagnosis, treatment, prescribing and management of patients, whilst promoting high standards of patient-centred care. The role operates across the four pillars of advanced practice: clinical practice, leadership and management, education, and research, contributing to service development, workforce transformation, and improved patient outcomes.

Main duties of the job

• Assess, diagnose and manage patients with acute illness, chronic disease and complex health needs.
• Undertake home visits for housebound patients and those requiring urgent assessment.
• Participate in triage processes, including reviewing Accurx and other digital consultations.
• Conduct clinical assessments of patients presenting with undifferentiated conditions.
• Develop, implement and evaluate evidence-based treatment plans.
• Diagnose and manage acute and long-term conditions using appropriate pharmacological and non-pharmacological interventions.
• Independently prescribe and review medication in line with clinical guidance, formularies and protocols.
• Monitor treatment effectiveness and amend management plans as required.
• Provide patients and carers with advice and education regarding diagnoses, treatments, medications and self-management.
• Support adherence to agreed treatment plans and health goals.
• Identify patients at risk of developing long-term conditions and implement preventative interventions.
• Recognise and respond to urgent and emergency presentations, initiating immediate care and escalation where necessary.
• Promote health, wellbeing and disease prevention through health promotion and lifestyle advice.
• Work collaboratively within the multidisciplinary team and maintain accurate clinical records.

Clinical Responsibilities:

Assess patients presenting with acute illness or for review of chronic disease conditions.

Undertake home visits for acute illness

Play a part in our triage team to assess and sign post patient requests received via AccuRX.

Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

Assess diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.

Work with patients and their carers to support compliance with and adherence to prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
