Job summary

We are looking for an enthusiastic and patient focused Registered Nurse to join our growing Clinical Homecare Nursing Team as a Community IV Nurse.

Working in partnership with NHS teams and healthcare professionals, you will deliver specialist IV therapies and Parenteral Nutrition (PN) to patients in their own homes, helping them receive safe and effective treatment while maintaining their independence.

This is a community based role requiring regular travel between patient visits. Travel between visits is included within your working hours, and a full UK driving licence is required.

Main duties of the job

As a Community IV Nurse, you will provide specialist nursing care to patients in their own homes, supporting them to safely receive treatment outside of the hospital environment.

You will manage your own patient visits, assess patients and deliver prescribed IV therapies and Parenteral Nutrition, while identifying and escalating any clinical concerns appropriately.

A key part of the role is helping patients become confident and independent with their treatment. You will provide education, guidance and reassurance to patients, families and carers throughout their homecare journey.

You will work closely with NHS teams, healthcare professionals and our wider Clinical Homecare Nursing Team to provide a safe and seamless transition from hospital to home and ensure continuity of care.

Community IV Nurse

As a Community IV Nurse within our Clinical Homecare Nursing Team you will deliver safe high-quality and patient-centred nursing care to patients in their own homes and community settings

Working in partnership with NHS teams and healthcare professionals you will support patients receiving specialist Parenteral Nutrition IV therapies and other prescribed clinical interventions at home

The role offers the opportunity to work autonomously while remaining part of a supportive wider clinical team

Key Responsibilities

Deliver safe high-quality patient-centred care in patients' homes and community settings

Administer prescribed Parenteral Nutrition IV therapies and clinical interventions in accordance with standard operating procedures professional standards and individual care plans

Undertake patient assessments monitor progress and identify changes in a patient's condition

Recognise and escalate clinical concerns appropriately supporting safe and effective patient care

Support the initiation of treatment ongoing care and the safe transition of patients from hospital to home

Educate train and empower patients families and carers to safely manage treatment and develop confidence and independence

Provide ongoing clinical advice guidance and support to patients throughout their homecare journey

Develop positive and professional relationships with patients families carers and healthcare professionals

Work collaboratively with NHS colleagues Senior Homecare Nurses Clinical Nurse Managers and wider healthcare teams to provide continuity of care

Maintain accurate timely and confidential clinical records in accordance with Clinical Governance requirements NMC standards and Inizio policies and procedures

Work in accordance with relevant clinical policies procedures professional standards and evidence-based practice

Contribute to patient safety quality improvement and the ongoing development of the Clinical Homecare Nursing service

Support the achievement of relevant service and programme performance measures

Maintain and develop clinical competence through mandatory training continuing professional development reflective practice and evidence-based learning

Community Working

This is a community-based position requiring regular travel to patients' homes

Visit locations and routes will vary according to patient needs and service demand

Travel between patient visits is included within your working hours

You will need to be comfortable travelling independently and hold a current valid UK driving licence

You will be responsible for managing your working day and patient visits effectively while maintaining close communication with the wider clinical team

Professional Responsibilities

As a Registered Nurse you will be expected to maintain your NMC registration and practise in accordance with the NMC Code

You will take responsibility for maintaining your professional knowledge and clinical competence and participate in relevant training development and revalidation activities
