Job summary

We are seeking an enthusiastic and experienced Band 6 Nurse to join our multidisciplinary team as a Frailty Practitioner. This is an exciting opportunity to work within the Weymouth and Portland Integrated Community Service to support Frailty Hospital At Home and the Complex Frailty MDT function.

The post holder will play a key role in delivering high-quality, person-centred care for patients living with frailty, recognising and managing complex/deteriorating patients and to prevent inappropriate admission to acute trusts.

This will involve working across community settings, providing specialist clinical input to optimise independence, reduce hospital admissions, and improve patient outcomes. You will act as an autonomous practitioner while working collaboratively within the Integrated Neighboured Team and wider services to deliver coordinated care.

You will contribute to the development of frailty pathways and support service improvement initiatives, ensuring care is evidence-based, safe, and responsive to the needs of the local population.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.

Main duties of the job

To deliver specialist frailty care and interventions, including assessment, treatment, and the management of a defined caseload as part of the multidisciplinary team.
