Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.

Working across a diverse patient population with a wide range of health and care needs, you'll help support people to remain safe, independent and well in their own homes. Community is at the heart of our Trust, and by working closely with local primary care partners we deliver responsive, patient-focused care tailored to the needs of each individual.

Main duties of the job
As a Band 3 Health Care Assistant, you'll work alongside community nursing colleagues to deliver person-centred care to patients in their own homes. Supporting people of all ages, you'll help implement care plans designed to promote independence, wellbeing and recovery.
Your patients may be older adults, people living with long-term conditions, individuals recently discharged from hospital, or those requiring end of life or disability support. You'll provide high quality care, monitor patient wellbeing, identify any changes in condition and share feedback with the wider team to ensure the best possible outcomes.
