Job summary

Kent and Medway Mental Health NHS Trust is seeking a dedicated Registered Mental Health Nurse, Occupational Therapist, Social Worker, or qualified CYP-IAPT practitioner to join our Canterbury CYPMHS team as a Multidisciplinary Pathway Worker.

In this role, you will deliver high-quality clinical assessments, formulation, care planning, and evidence-based interventions for children and young people up to age 18 who present with emotional, behavioural, neurodevelopmental, and mental health needs.

You will manage an allocated caseload within structured CYPMHS pathways, combining autonomous clinical practice with multi-agency working alongside education, social care, primary care, and voluntary-sector partners. Supported by regular clinical supervision and a welcoming multidisciplinary team, you will contribute directly to high standards of care, safeguarding, and service improvement.

Main duties of the job

• Clinical Assessment & Formulation: Undertake comprehensive assessments covering mental state, risk, safeguarding, and psychosocial needs to co-produce robust care plans and risk-management strategies with children, young people, and their families.
• Evidence-Based Interventions: Manage an allocated caseload and deliver child-centred, family-focused interventions aligned with NICE guidelines and CYP-IAPT principles (such as CBT, Systemic Family Practice, psychoeducation, and parent support).
• Outcome Monitoring: Use routine outcome measures to track therapeutic progress, review care plans, and ensure positive outcomes for service users.
• Multi-Agency Collaboration: Partner effectively with local schools, GPs, paediatric teams, social services, and third-sector organisations to deliver integrated support.
• Governance & Service Improvement: Participate actively in MDT meetings, clinical reviews, audits, safeguarding discussions, and service development initiatives.
• Team Support & Clinical Administration: Maintain accurate, timely electronic clinical records while supporting junior colleagues, trainees, or students and engaging in continuous professional development.

Job description

The post holder must hold a full, valid, UK driving licence to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Experience & Skills: Demonstrable skill in risk assessment, formulation, and delivering evidence-based therapies to children and young people. Strong communication and multi-agency working abilities.
