Job summary

This is an exciting opportunity to join the Ambulatory Wound Care and Lymphoedema Service, delivering specialist lymphoedema care. The service combines two interconnected pathways: Wound Care and specialist Lymphoedema care. We are seeking a motivated and experienced Band 6 practitioner to join our team.

The post holder will support the development of the service, delivering specialist lymphoedema care providing clinical leadership, supervision and guidance to Band 5 staff. The role will ensure safe, effective, high-quality care and contribute to a responsive, patient-focused service aligned with local and national standards.

Responsibilities include the assessment, planning, delivery and evaluation of care with lymphoedema, chronic oedema and lipoedema, including those with cancer and palliative care conditions. The post holder will manage a specialist independent caseload to provide holistic care in community clinics and patients' homes. The postholder will work with a multidisciplinary team and will contribute to care delivery across primary care, community services and specialist teams. The role also includes service development, clinical governance and audit.

The post holder will deliver education and training to patients, carers and healthcare professionals and promoting self-management. A recognised lymphoedema qualification is desirable; however, support will be provided for the successful candidate to undertake the relevant training and development within the role.

Main duties of the job

To manage a caseload of adult patients with all types of Lymphoedema using specialist skills and evidence-based, patient centred approach to assess, plan, implement and evaluate care.

To provide specialist support to the AWC nurse-led clinics and community nursing teams in Sheffield, ensuring a high standard of care in collaboration with the Tissue Viability, Vascular and Podiatry services.

To prescribe in line with the wound care formulary and to ensure that nurse prescribing is undertaken in a safe, cost-effective and evidence-based manner.

To provide support to the workforce with appraisals, clinical supervision, audit, education, learning and development.

To assist with planning, evaluation, and development of services, contributing to service improvement projects.

To participate in multidisciplinary team working and have an active role in joint clinics and home visits.

To work to deliver comprehensive, holistic care that is responsive to the patient population.

To contribute to the coordination of patient care alongside GP's, Oncologists, Surgeons, Specialist nurses and Community nursing teams.
