Main duties of the job
Duties will include -
- Manage repeat and acute medications
- Carry our structured medication reviews
- Provide support and education to patients on their medications, enabling them to get the most from their medications with the minimum number of side effects.
- Support the management of long-term conditions and can provide enhanced care for the following ( but this list is not exhaustive)
- Chronic kidney disease
- Metabolic syndrome
- Diabetes
- COPD
- Asthma
- Patients with cardiovascular risk

Job Description Clinical Pharmacist
Job Title: Clinical Pharmacist Location: Member practices forming Braintree Primary Care Network Reports To: PCN Business Manager/Operations Manager
Purpose of Role:
To enhance patient care through effective medication management, ensuring safe and optimal use of medicines within the PCN.
Key Responsibilities:
- Conduct thorough medication reviews for patients, especially those with chronic conditions or on multiple medications (polypharmacy).
- Provide expert advice on prescribing, including assessing the appropriateness of medications and potential interactions.
- Develop and implement standard operating procedures for medication management and safety.
- Collaborate with GPs and other healthcare professionals to create and adjust medication management plans.
- Participate in practice meetings to discuss medication-related issues and provide recommendations.
- Educate patients about their medications, emphasising adherence, side effects, and lifestyle considerations.
- Contribute to the development of local formularies and medication policies to align with best practices.
Qualifications:
- Registered pharmacist with the General Pharmaceutical Council (GPhC).
- Postgraduate qualification in clinical pharmacy or experience in primary care preferred.
- Strong analytical and communication skills.

Braintree Primary Care Network Ltd is seeking a motivated and enthusiastic Clinical Pharmacist to join our friendly multidisciplinary team. The successful candidate will play a key role providing support across our three member practices and two hosted practices, serving a combined population of approximately 58,000 patients.
Working closely with GPs, nurses, and the wider primary care team, you will help deliver high-quality, patient-centered care. You will be accountable to Braintree PCN Ltd and will primarily work in house at one of our member practices.
