Job summary
Woodbank Surgery in Brandlesholme, Bury are looking for a highly motivated and hardworking Advanced Nurse Practitioner to join their clinical team. The post holder will work collaboratively with the GPs, PNs and HCA to provide assessment, treatment and care to patients ranging from infants to elderly people.
The role is for 37.5 hours per week, to be worked Monday Friday. The practice is open until 8pm on Tuesdays and cover for this would be required. Salary is dependent on qualifications and experience. Annual leave allowance is 5 weeks + bank holidays. Enrolment to the NHS pension scheme is automatic.
Remuneration of 44K 52K WTE depending on experience
Main duties of the job
The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The post holder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.
The practice uses Vision, Docman mail system and askmyGP. Experience in these systems would be advantageous and good IT skills are essential.

Job Summary:
Responsible for the assessing, diagnosing, prescribing and treating of patients who are
registered within the GP practice (as well as for non registered patients from other practices
whom the practice has responsibility for through any of its enhanced services schemes).
The post holder will participate fully in the delivery of a high quality, co-ordinated health
care system delivery for patients catered for by the practice. There will also be
responsibility for training, supervision of and mentoring of the Nursing Team including
Practice Nurses, Healthcare Assistants
Principle Responsibilities:
A key member of the clinical team, the Nurse Practitioner will manage a caseload, dealing with a wide range of presenting healthcare problems and needs.
The Nurse Practitioner may see both self-referred or internally referred cases and make professional autonomous decisions in relation to their problems.
Patients with previously undifferentiated and undiagnosed problems will be seen and an assessment of their healthcare needs will be made.
Whilst making an assessment of healthcare needs the opportunity for disease risk management will also be recognised and taken.
A plan of care will be developed which is both individualised and patient focused. Implementation and evaluation of the plan of care will involve the patient and other members of the primary care team, where appropriate.
Preventative measure for achieving optimum healthcare gain will be emphasised.
Communicates current health care measures and strategies, for health education, to the patient in both a realistic and therapeutic manner.
Admits and discharges patients to and from a caseload and refers them to other care providers both internally and externally as appropriate.
The Nurse Practitioner, with an Independent Prescribing Qualification, will be expected to prescribe medication from the Extended Nurse Prescribing Formulary and in line with evidence based practice.
Managerial Responsibilities:
To offer support and advice to other members of the nursing team within the GP Practice.
To liaises with other members of the primary health care team/other agencies
To contribute to the Practice Business plan
To support the IPA and PDP process of other practice nurses
Clinical Governance:
To contribute to policy development and the implementation of procedures and guidelines in relation to own area of work and child protection.
To endeavor to ensure a high standard of evidence based care is delivered to all patients by working within role specific guidelines. At the same time the limitations of role specific guidelines will be recognised so allowing autonomous decisions still to be made where appropriate.
Ensuring effective teamwork with amendatory attendance at a minimum of 70% of the in-house educational and patient care meetings.

To mentor Practice Nurses and Healthcare Assistants
To support educational activities within the clinical setting, to ensure the delivery of high quality education.
To undertake clinicalaudit and research in line with service development and evidence based practice.
To liaise with other nurse practitioners within organisations to ensure the development of the role.
To attend any training that is identified as mandatory to their role.
Clinical:
Assessing Healthcare Needs: The nurse practitioner is equipped with the essential skills to assess health including history taking, examination and interpreting investigations
Anatomy and Physiology: Has the skills to consolidate understanding and apply scientific method to understanding the way the body functions
Pharmacological Intervention: Has a developed understanding of the nature and action, including pharmacokinetics and pharmacodynamics, which enable the nurse practitioner to prepare and educate patients about their medication.
Therapeutic Communication: Has developed therapeutic communication skills with regard to initial consultation, health promotion and collaborative care planning.
Context of Healthcare: Has an understanding of health and social policies in the United Kingdom and is able to explore the professional issues in an ever-changing system
Clinical Dissertation: Has undertaken and completed a clinical dissertation which has provided an understanding of the process of systematic reviews of literature and has included research appraisal and statistical synthesis.

Working within the Practice you may gain knowledge of confidential matters, which may include personal and medical information about patients and staff. Such information must be considered strictly confidential and must not be discussed or disclosed. Failure to observe this confidentiality could lead to disciplinary action being taken against you.
