Do you want to make a real difference in your local community? </em>If so, wed love to hear from you!</p><p>Join HCRG Care Group as a <strong>Healthy Family Practitioner</strong> based in Skelmersdale and play a vital role in supporting children, young people, and families across Lancashire.</p><p>Working as part of the <strong>Young People & Healthy Families Service</strong>, youll collaborate closely with Health Visitors, School Nurses, and wider multidisciplinary teams to deliver high-quality, compassionate care. This is a rewarding community-based role where youll empower families, promote child development, and help improve long-term health outcomes.</p><p>This is a part-time fixed-term opportunity working 30 hours per week with a planned start date of September 2026.</p><p><strong>What you\'ll be doing:</strong></p><p>In this varied and impactful position, you will:</p><ul><li>Deliver key elements of the <strong>Healthy Child Programme</strong>, including developmental reviews (e.g. 1-year and 22.5-year checks)</li><li>Support children and families with <strong>targeted interventions</strong> such as behaviour management and infant feeding</li&g
