Job summary

Stone & Eccleshall Primary Care Network is seeking a compassionate and motivated Wellbeing Practitioner / Social Prescribing Link Worker to join our multidisciplinary team. This is an exciting opportunity to help people access the support they need beyond traditional medical care, addressing the social, emotional and practical factors that impact health and wellbeing. Working across our PCN practices, you will empower individuals to take control of their wellbeing through personalised support and connections to local community services, voluntary organisations and statutory agencies.

Main duties of the job

Work directly with patients referred from GP practices and community partners.

Conduct person-centred wellbeing conversations and assessments.

Develop personalised support plans focused on what matters most to each individual.

Help people access community groups, social activities and practical support services.

Support individuals experiencing issues such as loneliness, financial difficulties, unemployment, housing concerns and emotional wellbeing challenges.

Build strong relationships with local voluntary, community and statutory organisations.

Maintain accurate records and contribute to service improvement initiatives.

Work collaboratively with GPs, practice teams and wider PCN colleagues

Job description

The Wellbeing Practitioner / Social Prescribing Link Worker plays a key role in empowering people to take control of their health and wellbeing through personalised, non-medical support. Working within Stone & Eccleshall Primary Care Network, the post holder will provide holistic, person-centred guidance, connecting individuals to community groups, voluntary sector organisations and statutory services that can support their wider social, emotional and everyday living needs. The role requires close collaboration with the network practices, PCN colleagues and a wide range of local partners. The postholder will be expected to work across the three network practices on a rota basis and will manage a varied caseload, with face to face appointments and development of personalised support plans. They will also support the development and sustainability of local VCSE provision and community groups.
