Job summary
CHC Senior Assessor is a highly experienced practitioner who provides advanced clinical expertise and leadership across the Continuing Healthcare (CHC) pathway.
The postholder takes responsibility for completing and overseeing complex CHC assessments, ensuring that eligibility recommendations are clinically robust, person-centred, and fully aligned to the National Framework. This role goes beyond frontline assessment.
The Senior Assessor supports practitioners, offers specialist guidance on difficult or ambiguous cases, and contributes to quality assurance across the service. They play a key part in strengthening documentation standards, improving clinical rationale within Decision Support Tools, and promoting consistent, defensible decision-making.
The postholder works closely with multidisciplinary teams, local authorities, providers, and system partners to ensure joined-up assessments and clear communication with individuals and families. They help navigate clinical, legal, and emotional complexity, supporting fair outcomes and maintaining confidence in the CHC process. As a senior clinician within the service, the Senior Assessor also contributes to training, capability building, and continuous improvement. They identify gaps in practice, support reflective learning, and help embed a high-quality, learning-focused culture across the CHC team.
Main duties of the job
oUndertake complex CHC assessments, completing high-quality Decision Support Tools, rationales, and recommendations in line with the National Framework.oProvide senior clinical advice and guidance to CHC practitioners on challenging or ambiguous cases.oQuality-assure assessment documentation, evidence gathering, and rationale writing to support defensible and consistent eligibility decisions.oSupport and contribute to CHC panels, MDT meetings, and case discussions, offering expert clinical input.oAct as a point of escalation for practitioners seeking support with clinical reasoning, risk issues, or complex presentations.oMentor and coach less experienced staff, helping build capability and confidence across the CHC team.oIdentify practice variation or quality concerns and contribute to targeted improvement activity.
