Job summary
An exciting opportunity has arisen for a suitably experienced candidate to join our busy multi-disciplinary team; working alongside paediatric and adult professionals to deliver care and support to children, young people and their families living with HIV. Close relationships will be required with the adult service and health advisors ensuring support from pregnancy through to young adulthood.
We offer support to children, young people and their families across the West Midlands Region and work closely within a National Network.
The successful candidate will have relevant experience in supporting children, young people and their families with complex social needs and a long-term health condition; in particular HIV.
HIV is recognised as a long-term, manageable health condition however the patient living with HIV requires support with physical, emotional and psychosocial needs. This role requires a candidate who is confident, enthusiastic and sensitive to the needs of families.

Main duties of the job
This role is part of a wider support team caring for our caseload of children and young people. You will work alongside our lead Clinical Nurse Specialist, and the MDT to ensure best practice and safe care.
Our successful candidate will be able to identify individual needs and develop a programme of targeted support or access to services designed to help children, young people and their families. This will include support with practical skills, direct work with children, liaison with professionals and working alongside health visitors, children's social care services, education and other agencies both statutory and voluntary. Support will be needed alongside appropriate referral to supportive agencies and charities.
This role requires knowledge of current asylum and immigration Policy, benefits available to families and understanding of local housing processes.
Excellent communication skills are essential as is being committed to delivering high-quality care to children in their own communities. Good team working and interpersonal skills are required as is an ability to manage your own time and lone work for periods of time.
The role is predominantly office based at Heartlands Hospital but will include community visits and lone working. Accurate electronic record keeping, management of clinical records, requesting relevant tests and clinical letters will be a part of the daily routine in this role; therefore, a good level of IT awareness and skill is required.
