Job summary

As a Care Co-ordinator, you will play a key role in supporting patients to access the right care at the right time. Working as part of the multidisciplinary practice team, you will coordinate care for patients with complex or long-term health needs, helping to improve their experience and health outcomes. The role involves liaising with healthcare professionals, community services, patients, and carers, maintaining accurate records, and ensuring care is well organised, person-centred, and delivered in line with NHS and practice standards. Main duties of the job

The Care Co-ordinator will support the delivery of high-quality, person-centred care by coordinating services for patients with complex or long-term health needs. Working closely with GPs, nurses, the wider multidisciplinary team, community services, and external agencies, the post holder will help ensure patients receive the right care at the right time.

Key responsibilities include coordinating patient care, monitoring referrals and care pathways, supporting multidisciplinary team meetings, maintaining accurate patient records, communicating effectively with patients and carers, and signposting patients to appropriate health, social care, and voluntary sector services. The Care Co-ordinator will also support proactive patient management, contribute to service improvement, and ensure all work is carried out in line with practice policies, NHS standards, information governance, and safeguarding requirements.
