Job summary

Barton Family Practice is looking for a dedicated, enthusiastic and compassionate General Practice Nurseto join our friendly and supportive team on a permanent basis.

The successful candidate will provide high-quality, patient-centred care, working closely with our GPs, nursing team and wider multidisciplinary team. This is a varied role offering the opportunity to develop your skills and make a real difference to the health and wellbeing of our patients.

Duties will include long-term condition management, immunisations and vaccinations, cervical screening, treatment room procedures and health promotion. You will also support patients with self-management and preventative care.

We are looking for a confident and motivated registered nurse with excellent communication and clinical skills. You will need to be organised, adaptable and able to work effectively both independently and as part of a team, while maintaining a high standard of safe and compassionate care.

Previous experience within general practice or primary care is essential.

Main duties of the job

  • Provide high-quality, evidence-based nursing care to patients within the practice.
  • Assess, monitor and manage patients with acute and long-term health conditions.
  • Undertake routine reviews and monitoring for conditions including diabetes, asthma, COPD, hypertension and cardiovascular disease.
  • Administer vaccinations and immunisations in line with national guidance and practice protocols.
  • Carry out cervical screening.
  • Undertake wound assessment, treatment and ongoing wound management.
  • Perform routine clinical observations, including blood pressure, pulse, oxygen saturation and other appropriate assessments.
  • Provide health promotion, lifestyle advice and education to support patients in managing their health.
  • Identify changes in a patient's condition and escalate concerns appropriately to GPs or other healthcare professionals.
  • Provide appropriate treatment, advice and signposting to patients, referring to other services where necessary.
  • Maintain accurate, contemporaneous and confidential patient records in accordance with professional and practice requirements.
  • Ensure safe infection prevention and control procedures are followed at all times.
  • Participate in clinical audits, quality improvement and practice development activities.
  • Maintain professional registration, clinical competence and continuing professional development.
  • Support the smooth running of the practice and contribute to the delivery of an efficient, patient-focused service.

Job description

The Practice Nurse will be responsible for providing high standards of clinical care, including:

  • Undertaking cervical screening and NHS Health Checks.
  • Providing wound assessment, treatment and ongoing wound management.
  • Carrying out treatment room procedures and other appropriate nursing interventions.
  • Providing health promotion, lifestyle advice and education to support patients in managing their health.
  • Supporting patients with preventative care and self-management.
  • Recognising changes in a patient's condition and escalating concerns appropriately to GPs or other healthcare professionals.
  • Providing appropriate advice, treatment and signposting to patients and referring to other services when required.
  • Maintaining accurate, contemporaneous and confidential patient records in accordance with professional and practice requirements.
  • Ensuring infection prevention and control procedures are followed at all times.
  • Working collaboratively with GPs, Healthcare Assistants, administrative colleagues and other members of the multidisciplinary team.
  • Participating in clinical audits, quality improvement initiatives and practice development.
  • Supporting the effective running of the treatment room and contributing to the delivery of an efficient and patient-focused service.
  • Maintaining professional registration, clinical competence and continuing professional development.
  • Keeping up to date with current clinical guidance, legislation and best practice relevant to the role.

The Practice Nurse will work closely with the wider practice team to ensure patients receive coordinated, safe and effective care. Good communication and a positive approach to teamwork are essential.

The postholder will be expected to communicate effectively with patients, relatives, carers and colleagues, treating everyone with dignity, respect and professionalism. The ability to work independently, use clinical judgement and recognise when further assessment or support is required is an important part of the role.
