Job summary

Are you an enthusiastic and motivated Occupational Therapist looking to develop your skills within a fast-paced community and urgent care environment? We are seeking a committed Band 6 Occupational Therapist to join our Discharge to Assess (D2A) Service, supporting individuals to leave hospital safely and continue their assessment and rehabilitation in their own home.

This is an exciting opportunity to work as part of an MDT focused on reducing hospital length of stay, promoting independence, and preventing unnecessary admissions and readmissions. You will play a key role in delivering person-centred assessments and interventions that enable people to achieve the best possible outcomes within their home environment.

Working alongside community providers of reablement services, Physiotherapists, Therapy support workers, District Nurses and Social Care asses as part of a MDT approach.

To work autonomously within the community, liaising with other health and social care professionals, to enable people to facilitate discharge from the community hospital to return to their homes in the community following illness or disability.

To provide high quality OT assessments, providing direct rehabilitation where necessary & managing patients with a range of conditions.

We have 2 posts available - 1 across the City-West localities & 1 across the City-East localities.

Main duties of the job

Complete holistic occupational therapy assessments within patients' own homes following hospital discharge. Assess functional abilities, activities of daily living, cognition, mobility, environmental factors and risks. Develop personalised rehabilitation and reablement plans that maximise independence and wellbeing. Recommend and arrange appropriate equipment, adaptations and support services.Work closely with hospital teams, primary care, community services, social care and voluntary sector partners to ensure seamless transitions of care. Support the prevention of avoidable hospital admissions and readmissions.Manage a varied caseload, prioritising effectively within a dynamic community setting. Provide supervision and support to junior staff, students and support workers where appropriate. Contribute to service development, quality improvement initiatives and integrated working across health and social care. You will need to be able to; triage cases and prioritise cases to effectively caseload manage, display an organised and disciplined manner, work remotely and autonomously, be decisive and a team player - maintaining a good team working rapport daily. Remotely supervise team members, display a passion for community rehabilitation.
