We are seeking a skilled, compassionate and enthusiastic Practice Nurse to join Modality East Surrey Medical Practice. Caring for approximately 45,000-46,000 patients across four sites, we offer the opportunity to deliver the breadth of practice nursing within an innovative, forward-thinking training practice. We are recruiting to a full-time equivalent post and welcome part-time applications.

Main duties of the job
You will assess, plan, deliver and evaluate evidence-based nursing care, combining long-term condition management with prevention and health promotion. Working within your competence and agreed protocols, you will support safe, coordinated care and contribute to audit and service improvement. Your work will include routine practice nursing procedures, personalised advice and timely escalation of clinical concerns.

The Practice Nurse will deliver safe, evidence-based nursing care across the East Surrey practice, working within NMC professional standards, demonstrated competence and agreed clinical protocols. Site allocation and any cross-site working arrangements will be discussed during recruitment and induction.
Core responsibilities include:
- Nurse-led reviews and support for long-term conditions, including diabetes, asthma and COPD, according to training and competence.
- Cervical screening, immunisations and vaccinations with appropriate training and authorisation.
- Wound assessment, dressings, suture removal and other routine nursing procedures within competence.
- Preventative care, screening and health checks, with personalised lifestyle and self-management advice.
- Recognition and escalation of deterioration and safeguarding concerns.
- Accurate clinical documentation, infection prevention and safe handling of medicines and equipment.
- Collaboration with specialist nurses, GP partners and the wider multidisciplinary team.
- Participation in audit, quality improvement, continuing professional development and sharing learning.
You will contribute to a supportive learning environment and the development of nursing services. Teaching, supervision and mentorship responsibilities will reflect your experience and training.

- Professional registration: Current NMC registration must be verified.
- Vaccinations: Mandatory vaccination evidence is required for this role, including Hepatitis B, MMR, and Varicella

- Right to work: Evidence of eligibility to work in the UK is required at interview stage.

Our sites are Birchwood and Wayside in Horley, Smallfield and Townhill in Caterham. Our experienced nursing team includes a Diabetes Specialist Nurse and a Respiratory Specialist Nurse. The wider clinical team includes two paediatric Advanced Nurse Practitioners working at consultant-level competency within their scope of practice. GP partners, healthcare assistants and operational colleagues support multidisciplinary working. We focus on accessible care, continuity, proactive long-term condition management and services for patients with complex needs. We use digital systems to support coordinated care and welcome staff involvement in developing pathways.
