Practice Nurse with Mentor Qualification | 18.75 hours per week | 18 month fixed term contract

Friar Gate Surgery is looking for an experienced and enthusiastic Practice Nurse with a recognised Mentor Qualification to join our friendly nursing team.

We are a long-standing and well-respected GP practice located close to Derby town centre, with parking available just off Friar Gate. We pride ourselves on providing a welcoming, high-quality GP service to all our patients.

The role will involve all aspects of practice nursing, including cervical screening, childhood immunisations, recording vaccinations administered abroad, family planning and chronic disease management.

We are particularly looking for an experienced Practice Nurse who can provide mentorship and support to our current Nurse Associate, who will be undertaking her Nursing top-up degree from January 2027. The successful candidate will play an important role in supporting her development, providing clinical guidance and helping her build on her skills and experience throughout her training.

You will work alongside our Nurse Associate who will be training to be a Practice Nurse and two GP Partners to provide high-quality, patient-centred care to our practice population.

We are looking for someone who is enthusiastic, compassionate and committed to delivering excellent patient care, with the experience, confidence and mentoring skills to work as part of our small and supportive team.

Main duties of the job

Patient Care: Assist and support with clinical tasks like wound care, cytology, childhood immunisations, medicals, and chaperoning. Conduct pathology requests, and manage LTCs including Diabetes, Asthma, COPD, and the other Quality and Outcome Framework Reviews.

Clinical & Admin Duties: Follow protocols, maintain equipment and rooms, ensure stock rotations, and handle vaccine orders manage flu home visits, keep mandatory check records, and support administrative processes.

Confidentiality: Safeguard patient data and uphold practice policies on sensitive information.

Health & Safety: Implement infection control protocols, manage PPE and hand hygiene, handle specimens, and oversee sterile environments. Train on safety measures, monitor equipment, and address hazards.

Equality & Diversity: Promote inclusive behaviour, respect patient rights, and act in a non-judgemental manner.

Development & Quality: Participate in training, personal development, performance reviews, and audits. Work on team and individual effectiveness, and ensure high standards of care.

Communication: Maintain clear, adaptable communication with patients, carers, and colleagues.

Clinical Skills

Long-term condition management:

  • Asthma (including peak flow readings and spirometry)
  • COPD
  • Diabetes - Reviews including foot checks and care planning
  • Hypertension, Atrial Fibrillation, Heart Failure, and CHD Management
  • CKD Management

Screening and health checks:

  • BP checks, height, weight, BMI, urinalysis, and smoking status
  • New Patient checks
  • NHS Health Checks
  • Mental Health Reviews
  • Learning Disability Reviews

Women's health and family planning:

  • Cervical cytology
  • General family planning services

Procedures and wound care:

  • Wound care
  • Suture and clip removal

Travel health:

  • Advice and preparation
  • Travel Immunisations
  • NHS Vaccinations and immunisations

Additional procedures:

  • Ear syringing

Clinical Responsibilities

  • Administer injections under an individualised prescription or PGD
  • Request pathology tests, such as urine cultures and swabs
  • Assist GPs with minor surgery and family planning as required
  • Maintain cold chain procedures to ensure safe storage, rotation, and disposal of vaccines and drugs

Infection Prevention and Control

Apply infection control measures within the practice in line with local and national guidelines, including:

  • Universal hygiene precautions
  • Collection and handling of laboratory specimens
  • Segregation and disposal of waste materials
  • Decontamination of instruments and clinical equipment
  • Reporting and treatment of sharps injuries
  • Managing blood and body fluid spillages
  • Ensuring safe disposal of clinical waste and PPE
  • Support patients and colleagues in adopting effective infection control measures
  • Lead or participate in infection prevention and control audits to maintain high standards

Emergency Preparedness

  • Be proficient in anaphylaxis and resuscitation techniques to respond to emergencies effectively
  • Support the maintenance of emergency equipment (e.g., trolley and drugs) and attend updates as required

General Duties

  • Maintain and ensure cleanliness of equipment used by nurses and GPs
  • Stock and rotate items in GP and nurse rooms as needed
  • Chaperone and assist patients during examinations conducted by other clinicians
  • Ensure the cleanliness and tidiness of treatment rooms

Administrative Tasks

Order and manage stock such as:

  • Health Promotion Literature
  • District Hospital supplies
  • Vaccinations to maintain adequate levels
  • Participate in administrative systems and ensure accurate documentation of patient medical records following legal and ethical standards
  • Advise management on potential problems or errors within assigned tasks
  • Attend and contribute to Practice meetings as needed

Patient Care and Outreach

  • Perform any other delegated duties appropriate to the role

Education and Research

  • Participate in research projects within the practice as required
  • Engage in the education and training of students from all disciplines
  • Assist in the induction of practice staff, with particular emphasis on nursing team members

Liaison and Professional Development

  • Maintain effective communication with other agencies and staff involved in patient care, ensuring confidentiality is respected
  • Participate in continued education through attendance at courses and study days to support professional development

At Friar Gate Surgery we have 2 GP Partners, Midwife and a Nurse Associate. We serve a population of 4,500 patients. Staff car parking facilities available.
