Job summary
This is an exciting opportunity for an experienced clinical healthcare professional looking to influence and shape community service delivery in a strategic leadership role. While predominantly non-clinical, the position requires a strong clinical background and a proven track record of delivering high-quality patient care, alongside substantial experience in leading teams, managing services, overseeing budgets, and driving workforce planning across Community Core and Specialist Services.
Working in partnership with Clinical Leads, the Service Manager, and the Locality Matron, you will play a pivotal role in leading service transformation, driving innovation, and implementing improvements to systems, processes, and models of care. You will help ensure the delivery of safe, effective, and high-quality commissioned community services that meet the needs of patients and communities.

Main duties of the job
Core Functions of the Community Team Leader role:
- A duty to ensure that managed staff undertake all relevant statutory and mandatory training in accordance with the Trust Mandatory Training Policy, as well as role specific competency training, and continuing professional development.
- Responsible for the identification and management of clinical/organisational risks within their area of responsibility through their involvement with a comprehensive risk assessment, action planning and monitoring process.
- Responsible for appropriate key performance indicators, including workforce and service delivery performance and accountability measures
- Ensure responsive recruitment processes and facilitate workforce access to health and wellbeing resources, to ensure effective recruitment and retention of staff.
