This is a part-time position of 22.5 hours per week, worked over 3 days between Monday and Friday, with shifts from 8:00am to 4:00pm.

Trinity Medical Centre is looking for a Clinical Advisor to join our friendly and supportive team. This is an on-site role based at Trinity Medical Centre in Wakefield, providing telephone consultations to our registered patients. You will use your clinical experience and professional judgement to support patients with a wide range of healthcare needs, while working as part of an established and collaborative team.

Main duties of the job

Working on site at Trinity Medical Centre, you will provide telephone consultations to patients, helping them understand their symptoms and ensuring they receive appropriate and timely care.

Your day will involve speaking with patients by telephone, gathering relevant information, assessing their needs and deciding on the most appropriate next steps. You'll need to recognise when a patient requires urgent or emergency support, while also being confident providing advice, reassurance and self-care guidance where appropriate.

You'll work with a wide variety of patients and presentations, so every day will be different.

Although working independently within your clinical competence, there is always a GP working alongside who is available to provide clinical support and advice when needed.

The role requires someone who can work calmly and confidently, manage their workload effectively and maintain a consistently professional and patient-focused approach.

Line Manager: Trinity Medical Centre, Nurse Manager Professionally Accountable to: Nurse Manager

Job Purpose

The post holder will be responsible for delivering a telephone clinical advice service to registered patients of Trinity Medical Centre. Patients will call with a wide spectrum of clinical conditions.

The post holder will:

Provide evidence-based clinical assessment, prioritisation and advice by telephone within clinical capabilities and competencies.Assess clinical risk through comprehensive telephone clinical assessments using Trinity Medical Centre protocols and professional clinical judgement.

Undertake comprehensive assessments using approved clinical judgement, assessment tools, evidence-based protocols and guidelines whilst working in Urgent/Primary Care.

Process calls in accordance with Trinity Medical Centre guidelines and provide a professional, courteous and efficient telephone consultation service.

Triage patients effectively, identifying those with emergency or urgent care needs, and provide routine care or self-care advice as appropriate.

Use critical thinking and effective communication skills to assess patients holistic needs, taking relevant cultural, social and economic factors into consideration.

Provide appropriate health education and advice, referring to the relevant healthcare professional as required.

Identify and use appropriate information sources to support clinical decision-making.

Maintain awareness of policies relating to mental health, child protection, vulnerable adults, medication enquiries and other aspects of general healthcare.

Take responsibility and accountability for managing own workload, risk assessment and risk management.

Work independently within the constraints of professional and organisational policies and guidelines.

Work effectively as part of a team, supporting and communicating with colleagues and clinicians.

Ensure consultations conclude with an agreed and safe clinical outcome and that accurate records are maintained.

Professional Responsibilities

The post holder will participate in audit, evaluation, feedback, clinical governance and performance review, comply with NMC/HCPC professional standards, maintain confidentiality and undertake continuing professional development relevant to the role.

Communication

The role requires excellent communication skills, including advanced listening, probing and facilitative skills. The post holder will communicate effectively with patients, colleagues and external services, adapting communication to individual needs and using tact and empathy, including when dealing with challenging or emotive calls.

Education, Training and Development

The post holder will attend mandatory and relevant training, maintain a professional development portfolio, participate in regular performance reviews and support the development of new staff through supervision, teaching and mentorship where required.

General

The post holder will work flexibly to support service needs and comply with Trinity Medical Centre policies, confidentiality requirements, UK GDPR, the Data Protection Act 2018, Equal Opportunities and Health & Safety requirements.

Safeguarding

The post holder has a duty of care to safeguard and promote the welfare of children and vulnerable adults, including recognising, responding to, appropriately referring and accurately documenting safeguarding concerns in accordance with organisational policies and procedures.
