An exciting opportunity has arisen on our Acute Frailty Unit (AFU) for a full time Clinical Support Worker to join our team.

A Clinical Support Worker (CSW) is responsible for providing healthcare to patients and supporting with clinical duties under the supervision of Registered Nurses, doctors and other registered healthcare professionals.

Their role is essential in ensuring the well-being and comfort of patients during their hospital admission ensuring they receive high quality care.

AFU is a 19 bed acute assessment unit for over 74s who have clinical frailty and associated conditions. Our aim is to view our patients from a MDT perspective to create the best possible care plan and safest discharge. The ward also has an SDEC area (Same day emergency care) which runs 7 days a week currently and has a capacity of 12 patients.

We liaise with the community services, such as UCR and Age UK as well as providing a Rapid Access clinic and Virtual Frailty Ward to monitor patients out of hospital.

Main duties of the job

• Provide and support patients as necessary with delivering both personal and clinical care activities relevant to training.
• Maintain accurate records of the patient's care, documenting as appropriate on the Electronic Patient Record (Cerner) in line with Trust, professional and departmental guidelines.
• Observe patients and reports any changes to a Registered professional for review.
• Report any abnormal findings directly to the registered professional in charge or doctor.
• Undertake a range of basic housekeeping, stock control and administrative duties within the clinical environment.
