Sarum North PCN is recruiting an Advanced Clinical Practitioner to work at St Melor House Surgery, Amesbury for 25 hours per week including a Monday evening for Extended Hours.  Come and join our small, patient focused team as an autonomous practitioner supporting the clinical team.

Main duties will be to assess and triage patients, including same day triage, and as appropriate, provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
