Job summary

Reporting to the UMAC Ward Manager, the Acute Take Coordinator is an experienced Registered Nurse who provides visible senior nursing leadership and clinical coordination for the acute medical take. The post holder works within the NMC Code and Trust policies to support the delivery of safe, effective, and compassionate care for all patients referred to medical services.

Positioned at the centre of the acute admission pathway, the Acute Take Coordinator maintains a real-time nursing and clinical overview of all medical patients from the point of referral--whether from the Emergency Department, SDEC, or other services--through to assessment, admission, and transfer to the appropriate clinical area.

The role requires advanced nursing assessment skills, clinical prioritisation, and early recognition of deterioration to ensure patients are placed safely and appropriately.

Main duties of the job

The post holder is responsible for ensuring that all patients are identified, clinically risk-assessed, and tracked throughout the admission pathway, escalating concerns in line with NEWS2, Trust escalation policies, and specialty pathways.

Working in close collaboration with Patient Flow, ED, UMAC, AMU, AFU, inpatient wards, and the wider multidisciplinary team, the role supports safe patient flow, reduces delays, and contributes to meeting national performance standards while maintaining patient dignity, safety, and experience.
