Job summary
Westside Medical Centre is seeking an enthusiastic and patient-focused Practice Nurse to join our friendly and supportive multidisciplinary team in Rugby, Warwickshire.
This is an excellent opportunity for an experienced General Practice Nurse to work within a busy and progressive GP practice, delivering high-quality clinical care, health promotion and long-term condition management to our patient population.
The post is offered on a permanent basis for 3.5-4 days per week (0.7-0.8 FTE), with Tuesday and Wednesday working essential. Salary is competitive and dependent upon experience.
Main duties of the job
The successful candidate will provide a range of general practice nursing services including:
- Chronic disease management, including asthma, COPD, diabetes and hypertension
- Cervical screening and women's health services
- Childhood and adult immunisations and vaccinations
- Wound care, dressings and treatment room procedures
- Health promotion and preventative healthcare
- NHS Health Checks and health screening
- Supporting patients to self-manage long-term conditions
- Working collaboratively within the multidisciplinary team to deliver safe, effective and patient-centred care
The successful candidate will hold current NMC registration and have experience working within General Practice or Primary Care.

Key Responsibilities
Clinical Responsibilities
The Practice Nurse will -
Deliver a high standard of patient-centred nursing care.
Assess, plan, implement and evaluate programmes of care.
Undertake clinical assessment of patients presenting with identified healthcare needs.
Provide advice and support to patients and carers.
Refer patients appropriately to other healthcare professionals and services.
Work within professional competence and scope of practice.
Maintain accurate, contemporaneous and legally compliant records.
Ensure informed consent is obtained where appropriate.
Maintain patient confidentiality at all times.
Long-Term Condition Management
Participate in the assessment and management of patients with long-term conditions.
Undertake reviews for patients with:
- Diabetes
- Asthma
- COPD
- Hypertension
- Coronary Heart Disease
- Chronic Kidney Disease
Promote self-management and healthy lifestyle choices.
Support achievement of QOF and other contractual requirements.
Monitor patients and escalate concerns appropriately
Cervical Screening and Women's Health
Undertake cervical screening in accordance with national guidance.
Promote participation in screening programmes.
Provide advice relating to women's health issues within competence and training.
Immunisations and Vaccinations
Deliver childhood and adult immunisations and vaccinations.
Administer vaccines in accordance with national guidance, PGDs, PSDs and practice protocols.
Maintain vaccine cold chain requirements.
Monitor vaccine supplies and report any issues appropriately.
Encourage vaccine uptake amongst eligible patient groups.
Treatment Room Services
Undertake wound management and dressings.
Remove sutures and clips.
Perform ear irrigation where appropriate.
Undertake venepuncture.
Record ECGs.
Perform spirometry where trained and competent ( if available )
Provide travel health advice and vaccinations where appropriately trained.
Carry out other treatment room procedures within competence.
Health Promotion and Prevention
Provide health education and health promotion advice.
Support smoking cessation initiatives.
Promote healthy lifestyle choices.
Support national and local screening programmes.
Encourage patients to engage in preventative healthcare.
Safeguarding
Recognise and respond to safeguarding concerns involving children and adults.
Follow local safeguarding procedures and practice policies.
Report concerns appropriately and in a timely manner.
Maintain required safeguarding training.
Infection Prevention and Control
Adhere to infection prevention and control policies and procedures.
Maintain safe standards of clinical practice.
Support infection control audits where required.
Ensure safe disposal of clinical waste and sharps.
Clinical Governance and Quality
Participate in clinical audit and quality improvement initiatives.
Contribute to incident reporting and significant event reviews.
Maintain awareness of current evidence-based practice.
Participate in practice meetings and clinical updates.
Emergency Care
Recognise and respond appropriately to medical emergencies.
Undertake annual basic life support training.
Participate in emergency response procedures within the practice.
Information Governance
Comply with UK GDPR and Data Protection legislation.
Maintain patient confidentiality.
Ensure records are maintained accurately and securely.
Complete mandatory information governance training.
Professional Responsibilities
Maintain current NMC registration.
Practise in accordance with the NMC Code.
Participate in annual appraisal and personal development reviews.
Maintain continuing professional development and professional competence.
Meet NMC revalidation requirements.
Work within professional boundaries and scope of practice.
Promote equality, diversity and inclusion.
Health and Safety
Comply with all health and safety policies and procedures.
Report accidents, incidents and hazards.
Ensure safe use of equipment and clinical areas.
Participate in mandatory health and safety training.
General Responsibilities
The post holder will:
Work collaboratively with the multidisciplinary team.
Participate in practice meetings and training activities.
Support practice objectives and service development.
Undertake any other reasonable duties commensurate with the role.
