Job summary

We have the opportunity to advertise for a Children & Young Persons Haemophilia and bleeding disorder clinical nurse specialist (CNS) to join our expanding Haemostasis Team within UHD. There is an ambition for UHD to become a bleeding disorder comprehensive care centre which will exemplify the high standard of care provided at the Trust. The service cares for patients of all ages including children, and provides cover for the whole of Dorset. Currently UHD is part of a Southern Haemophilia Network with our comprehensive care centre based in Basingstoke.

This role will work alongside our current Haemophilia CNS across Poole and Bournemouth sites and haematology consultants and is a fixed term contract for 2 years.

We are looking for an experienced paediatric trained nurse who has experience working with bleeding disorders and is highly organised, motivated to learn, with excellent communication and team working skills.

Main duties of the job

Provide a comprehensive specialist service toourchildrenand young people as well as our adult patients withhaemophilia andotherbleeding disorders.

Provide expert care, specialistadviceand support (financial, social, and psychological) to patients withbleeding disorders, their families, and carers.

Include a holistic patient assessment,planningand implementing specialist care.

Actas the key worker for patients from investigation toinitialdiagnosis and throughout the whole pathway to ensure coordination and continuity of care.

Liaise with all members of the multi-disciplinary team, including members of the Primary Health Care Team, to provide specialist nursing knowledge andexpertise

Bepassionate about providing high-quality care, have a keen interest in haematology, andpossessexcellent communication skills along with the knowledge and skills to be able to support patients through their pathways.

Job description

Work ismainly anoutpatient-basedservice(across both RoyalBournemouth& Poole Hospitals)including care within routine clinics, emergency walk ins to the haematology day unit/ Childrens Assessment unit and community visits. There will also be a need to provide care and support for in-patients.

Be availabledailyto support patients throughthe pathway

Develop and deliver a coordinated specialist service for patients, involving direct clinical care in both outpatient and inpatient settings

Arrange andfacilitatediscussions at multi-disciplinary team meetings toestablisheffective management plans for patients

Liaise with GPs

Serve as a specialist resource, offering advice and support to healthcare professionals and others involved in patient care

Assess the complex needs of patients and their families/carers, planning, implementing, and evaluatingappropriate careprograms. This includes communicating sensitive informationregardingdiagnosisandtreatment options

Provide emotional, psychological, and practical support to patients and their families/carers throughout thepathwayjourney, ensuring effective communication among patients, families, and healthcare professionals

Ensure the availability of specialist information for patients and their families/carers

Ensure clinical practice is effective, evidence-based, and focused on improving care quality and patient outcomes, with measurable standards and results

• Recognise and address ethical dilemmas in care, advocating for patients and their relatives when necessary

Proactively manage changes within the specialty to enhance practice and health outcomes for patients

Attend and contribute effectively to relevant professional forums

Assistwith research and development initiatives to advance the field

• Workwith autonomy with support from the adult haemophilia CNS and wider team

This current children's service has a planned move from Poolesite to Bournemouthsite in 2027.

If a role or service relocates as part of a planned move, excess mileage will not be reimbursed. Travel from home to the new work base will be classed as a normal commute. Any other changes will be managed under Trust or national terms and conditions.
