Job summary

An exciting opportunity has arisen to work within Haslington Surgery. We are seeking a motivated, innovative Care Coordinator, ideally with recent experience of the same role, in a Primary Care setting, or a good knowledge and understanding of clinical I.T. systems, having worked in a similar health care role. You will work closely with the clinical and administration teams to help and support the ongoing patient case management. This will involve linking Haslington Surgery with a range of Community Health Services. You will support the Haslington Surgery team to deliver effective, co-ordinated care to all patients. Your role will involve working proactively, with one other Care Coordinator, driving service delivery.

We are looking for someone who is hardworking, organised with great interpersonal skills who will be committed to delivering the highest quality of care for our Surgery. If you have what it takes and have the experience we would like to hear from you.

Working hours: Tuesday 14:00-18:30; Weds 14:00-18:30, Thursday 10:30-18:30; Friday 10:30-18:30.

Working hours: 24 hrs/ week.

Main duties of the job

• To support the admin team with Urgent and Non urgent referrals.
• Involvement in patient tasks and email responses.
• Management of the patient Home Visit request book.
• Act as Patient Complaint first point of contact.
• Work closely with the management team, to prioritise clinical team workload.
• To make effective use of the Clinical Team appointments book, contacting patients using all IT packages.
• Processing of patient records.
• Arranging clinical meetings and providing meeting minutes.
• Taking ownership of patient information boards and screens.

Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care.

Support patients to utilise decision aids in preparation for a shared decision-making conversation.

Holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person.

Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.

Support people to take up training and employment, and to access appropriate benefits where eligible.

Support people to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing, including through the use of the Patient Activation Measure.

Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.

Explore and assist people to access personal health budgets where appropriate.

Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals.

Support the coordination and delivery of MDTs.

About us

Haslington Surgery is a two Partner practice, located in a small village in Cheshire. We have two salaried GP's, four Nurses and one GPA. This is supported by a Practice Manager and Senior Receptionist working with a small administration and reception team. We form part of the local SMASH PCN group of Surgeries, working together to focus on local patient care and to deliver local services.

Together we are a friendly team who work closely and meet on a regular basis to promote continuous learning and professional development for all.

We are supportive of stretch and challenge, of all team members and roles.

More recently we have been involved with the training of new Nurses, with the Cheshire and Merseyside training hub.

Our patient population is rapidly expanding, due to new housing developments in the local area, and we are currently in the process of extending the surgery to meet these needs.
