PCN NurseGP Practice

Dartford Central Primary Care Network is looking for an experienced, compassionate and motivated Nurse to work in frailty, to join our team and help us develop our work through the Single Neighbourhood (SNH) programme.

This is a new and varied role focused on improving the care and experience of people living with frailty, particularly those whose health and circumstances place them at greater risk of deterioration, loss of independence or unplanned hospital admission.

Rather than waiting for patients to reach crisis point, you will work proactively with individuals, their families and the wider multidisciplinary team to identify needs early, coordinate care and put appropriate plans and support in place.

Your work will take you across our GP practices and potentially into patients’ own homes and into care home settings. You will have the opportunity to build strong relationships with patients and carers while working closely with colleagues across primary, community, social and specialist care.

The role would particularly suit a nurse who is confident working autonomously, enjoys managing complex clinical needs and has a genuine interest in improving care for older people and those living with frailty.
