The Care Transfer Hub North is looking for a B4 Assistant Practitioner / Nursing Associate to join our team on a fixed term basis, to cover maternity leave.

We are employed by Community but based on the acute hospital site at NDDH. Our remit is to assess and procure onward care for patients being discharged from hospital on discharge pathways 2 and 3; however, on occasions we may also support with pathway 1.

You will work independently but under the supervision of the clinical team to complete assessments for appropriate patient groups, in order to evidence eligibility for on-going care and to determine the type of on-going care they require. You will do this by visiting patients on in-patient wards to complete the necessary assessments, liaising with the ward MDT, liaising with them and their NOK to share necessary information and then producing a Transfer of Care assessment document which evidences their on-going needs.

The assessment you complete will then be used to source the care that the person requires and you will work closely with other relevant acute and community Health and Social Care teams to ensure the person is then discharged safely and in a timely way.

Whilst the role can be challenging at times, you will be well supported by a friendly team of Health and Social Care professionals. You will have the opportunity to gain valuable transferrable skills, in this unique team which works across acute, community and the private provider settings.

Working Pattern: 0800-1800, 4 days per week
