We are looking for an experienced and enthusiastic independently prescribing pharmacist to join our expanding medicines management team at Wide Way Medical Centre. We currently employ 1 independently prescribing pharmacist supported by a pharmacy technician, but recognise we need to expand our team to meet the needs of our 16,000 population.

We serve a diverse population in a relatively deprived area of Merton with a high burden of cardiovascular disease in particular so an interest in cardiovascular-disease care is desired but not essential.

The role is practice based and there is flexibility for hybrid working with a mix of on-site and remote work - this is currently largely dictated by physical room space constraints but is negotiable. For direct patient contact activity we like it to be face-to-face because we believe this builds better trust and rapport but non-direct activities can be performed remotely to an extent, but it is important that you meet colleagues and feel fully embedded as part of the whole practice team.

Main duties of the job

Medicines Management Responsible for implementing and communicating medicines-related policies, procedures, and protocols in line with SWL optimisation priorities. This includes repeat prescribing, ensuring appropriate and monitored medications, reconciling changes from external providers, and conducting structured medication reviews (SMRs). Also includes oversight of Personally Administered Items (PAIs), medicines audits, safety alerts, and responding to medicine-related queries. Liaison with community pharmacists, ICS and PCN teams is essential to deliver national/local priorities. Staff are updated on medicines performance, with quality improvement activities supported.

Long Term Conditions Management (CVD) As part of the Planned Care team, responsibilities include managing long-term conditionsfocusing on CVD, AF, Heart Failure, Diabetes, Hypertension, and CKDvia structured reviews and optimisation clinics. The role supports multidisciplinary collaboration to improve outcomes for high-risk patient cohorts.

Clinical Pharmacist Core ResponsibilitiesThe Clinical Pharmacist plays a vital role within the practice, contributing to patient care and medicines optimisation. The following outline the core duties, though additional tasks may be required depending on service demand, staffing levels, and operational needs:

1. Clinical Responsibilities

• Act as an independent prescriber, working safely and effectively within scope of practice.
• Conduct face-to-face and telephone consultations, including managing minor illnesses.
• Deliver structured medication reviews, both in-practice and during domiciliary visits (e.g. care homes).
• Provide advice and support for patients with self-limiting conditions.
• Promote and encourage cost-effective, evidence-based prescribing across the practice.
• Reconcile medication following hospital discharges or specialist clinic appointments.
• Review and manage medications for newly registered patients.
• Support clinicians with patients requiring substance misuse or dependency care.
• Undertake influenza and travel vaccinations within scope of training and protocol.

2. Medicines Optimisation & Safety

• Review and act upon medicines safety alerts, ensuring timely implementation of necessary actions.
• Lead on repeat prescribing, embedding robust systems and safe processes.
• Reauthorise repeat prescriptions within scope and identify patients suitable for inclusion in the repeat prescribing programme.
• Implement and monitor medication safety protocols, particularly for high-risk and vulnerable patients, including those on complex regimens.
• Conduct targeted medication reviews for patients at clinical risk (e.g. polypharmacy, frailty, long-term conditions).
• Provide subject matter expertise on medication monitoring and ensure effective systems are in place.

3. Practice Support & Development

• Assist the practice in achieving Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF) targets.
• Proactively signpost patients to the appropriate healthcare professional or service.
• Collaborate with the wider multidisciplinary team to support delivery of safe, effective and patient-centred care.
• Contribute to training, audit, and quality improvement initiatives related to prescribing and medicines use.

Wide Way Medical Centre is based in South West London.

We take care of almost 16000 patients in a modern, purpose built surgery with ample car parking.

Our team consists of 4 GP Partners plus a Managing Partner, 5 Salaried GPs, 2 Advanced Care Practitioners, Practice Nurses, a Paramedic, Social Prescriber, Health and Wellbeing Coach, Care Coordinators and a fantastic administrative support team.

We are a training practice with 4 GP trainers and we host GP registrars, medical students and nursing students.
