Job Title: Digital Transformation Lead
Responsible to: PCN Clinical Director
Accountable to: North Lambeth PCN Clinical Lead/GP Leads
Contract Length: 24 months initially
Hours: Up to 22.5 hours
Salary: £53,000 - £57,000 WTE
Main duties of the job
The PCN Digital Transformation Lead will:
- Develop and deliver the PCN Digital Transformation Strategy in liaison with staff and system stakeholders (e.g. local councils / Social Services).
- Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity.
- Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients, including shared appointments between practices to aid delivery of enhanced access.
- Lead an improvement approach to change, including building capability for quality improvement within the PCN and system-wide approaches to problem solving.
Job description
Increasing Patient Access via Digital Means
- Build relationships and collaboration between Practices, PCNs and the wider system, to support the delivery of care to patients. For example, delivery of enhanced access and shared appointments across the PCN and practices.
- Improve adoption and/or optimisation of new technology to enhance patient access and increase PCN productivity.
- Lead on improvements around building capability and quality within the PCN, including system-wide approaches to problem solving.
- Review and improve the PCN's Digital Maturity.
- Use data and improve data quality to:
- Understand demand, capacity and activity, and drive improvements.
- Review data on patient experience, including access to GP services.
- Review data and feedback from General Practice staff around experiences at work.
- Meet targets set by NHS England, the ICG/ICS and Government.
- Support Population Health Management by providing full-cycle Digital Project Management to the PCN.
- Support training needs around digital working for staff with the PCN team and wider practice staff.
- Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at network and practice level to improve access to services for patients, e.g. Ardens.
Service Improvements
- Responsible for the delivery of robust information systems to ensure that all project documentation is stored and used effectively (PID, dashboards, highlight reports, risk and issues logs etc.).
- Develop programme and project management tools, reports and templates that are transferable between projects and can be used for the whole project lifetime, from design to delivery.
- Alongside the Digital Care Co-Ordinator, act as a champion for patients and their interests and involve the public and patients in policy development and decision-making.
- Ensure all public and patient contact with the office is of the highest professional standard.
- Be a champion for best practice, learning from experience and from others, supporting the spread of innovation within the organisation.
Analysis and Communication
- Interrogate and analyse information to support clear project reporting and project outcomes EMIS/Ardens Searches, Health/population data, EZ Analytics.
- Write and submit regular project reports as required, showing progress against milestones and targets and making recommendations where appropriate.
- Where required, create KPIs so that we are able to meet milestones successfully.
- Communicate, engage and build relationships with key strategic policy makers to influence the development of strategy and policies.
- Communicate the vision for the PCN and support the development of strategy and operational policies that support this vision.
- Forge close working relationships across the PCN to facilitate an effective matrix-style approach to delivery of projects and programmes of work.
- Provide line management support for the Digital Team.
Collaborative Working Relationships
- Work as a cohesive team with the staff in the PCN and all six practices to develop and deliver appropriate digital projects.
- Actively develop and maintain effective working relationships with NHS stakeholders, e.g. NHS Digital, CSU, ICS IT Leads, Patient Participation Groups etc., and external stakeholders, e.g. local councils and voluntary organisations.
Confidentiality
- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
- The post-holder should always respect patient confidentiality and not divulge patient information unless sanctioned by the requirements of the role.
Continuous Professional Development
- The post-holder will undertake regular professional development as agreed for the role, taking an active part in reviewing and developing the role and responsibilities and providing evidence of learning activity as required.
Health and Safety / Risk Management
- The post-holder must comply at all times with the PCN and Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.
- The post-holder will comply with the Data Protection Act (1984), the General Data Protection Regulations (2018) and the Access to Health Records Act (1990).
Risk Management
- Responsibility for ensuring that risks are managed at work group and local level. This process will include the assessment and effective control of all acceptable risk situations.
- As an employee, you will be expected to maintain a high level of awareness and assist in the process of reporting incidents, assessing risks and reporting unsafe occurrences and co-operate with any investigations undertaken.
- Employees will be required to adhere to all employer policies and procedures and attend statutory and mandatory training.

Special Working Conditions
- The post-holder is required to travel independently between practice sites and other local venues and to attend meetings hosted by other agencies.
- The post-holder may also be required to attend virtual meetings and/or work from home.
Miscellaneous
- Demonstrate a flexible attitude and be prepared to carry out other duties as may reasonably be required within the general character of the post, ensuring that work is delivered in a timely and effective manner.
- Other duties which may be decided upon by the North Lambeth PCN Board from time to time.
- Work in accordance with the PCN policies and procedures.
- Contribute to the wider aims and objectives of the PCN to improve and support primary care.

Hurley and Riverside Practices are responsible for providing payroll, financial and HR support to the PCN as required. It will be an important part of the manager's role to liaise with these departments.
