Job summary

To work within the Department of Respiratory Medicine and Oncology at the Shrewsbury and Telford Hospital NHS Trust to support the further development of the service for patients who have Lung cancer and Mesothelioma. The post holder will work with the Lung MDT to develop deliver the service to reflect the needs of patients with Lung cancer and Mesothelioma and benign pleural pathologies throughout the patient pathway. The post holder will ensure the delivery of a high quality, clinically effective care, prioritising, and utilising research.

Main duties of the job

1. Act as the patient's key worker, providing advice, support and information at key stages, including complex treatment or clinical management decisions.

2. Undertake holistic assessment of physical, psychological and social needs, coordinating care and referrals to appropriate services.

3. Provide emotional and psychological support, signposting or referring patients for level 3 or 4 support where required.

4. Support Manual of Cancer Standards compliance, ensuring key worker access, records and information on diagnosis, treatment, prognosis, benefits and support services.

5. Work with specialist nurses, community teams, hospital services and the MDT to support seamless transitions of care.

6. Contribute to Lung MDT discussions as a patient advocate, supporting coordinated clinical decision-making.

7. Communicate effectively with patients and carers, providing information in appropriate formats.

8. Support the Pleural service with Respiratory Physicians, empowering patients in decision-making, symptom management, self-care and educating patients and staff.

9. Act as a clinical resource for patients, carers and staff, providing care, information and support throughout the pathway.

10. Coordinate a caseload from pre-pleural procedure to ongoing advice and community support.

Provide clinical support and coordination for the pleural service, acting as contact for Trust and community teams re pleural catheters and liaising with primary care after discharge.
