Job summary

Location:

• Key focus on working within local Cambridgeshire communities, with some home working.
• Based at EACH's Milton Hospice (CB24 6AB) for meetings, training, and team activities as required.

Salary:£25,700 - £27,500 per annum, dependent on experience

Hours:

• Full time, 37.5 hours per week (Monday Friday)
• Job share applications will be considered. Please indicate in your application if you wish to apply on this basis. For part-time hours, salary will be pro rata.
• Please note that flexibility to work occasional evenings and weekends is required.

Contract:Permanent

*A current full, valid driving license and access to own vehicle is essential for this role*

At East Anglia's Children's Hospices (EACH), we support children and young people with life threatening conditions, and their families, to receive care where and when they need it.

We are strengthening how we work with communities to ensure more families can access support earlier, feel connected, and receive care in a way that reflects their individual needs.

We are looking for a Community Connector to help build meaningful relationships across our communities, improve access to our services, and support inclusive, communitybased approaches to care.

This is an opportunity to play a key role in developing connections between EACH and the communities we serve.

Main duties of the job

You will:

• Build relationships with local communities, organisations and partners to increase awareness of EACH services
• Work alongside underrepresented communities to support more equitable access to care
• Support outreach activity, events and communitybased initiatives across the region
• Work with families and partners to coproduce inclusive engagement opportunities
• Provide information, advice and signposting to families and professionals
• Contribute to developing approaches that enable care beyond hospice settings

You will also work closely with colleagues across care, volunteering and fundraising teams to ensure a joined up and coordinated approach.

About you

Job description

• Proactively build and maintain relationships with local communities, organisations and partners to promote awareness and understanding of EACH services.
• Identify and engage with under-represented groups and communities who may face inequalities in accessing hospice care.
• Plan and deliver community engagement activities, including events, group work and wellbeing initiatives.
• Facilitate or support community groups that enable peer connection and inclusion.
• Map community networks and identify opportunities for partnership working.
• Support and deliver community-based engagement opportunities and local events across the hospice catchment area.
• Work in partnership with families, communities and external organisations to co-produce inclusive and meaningful engagement opportunities.
• Ensure the voices and experiences of families and communities inform engagement approaches and contribute to service development.
• Build effective partnerships with voluntary, community and statutory organisations.
• Support the development and delivery of community-based programmes, group work and wellbeing activities.
• Support opportunities that enable connection, peer support and a sense of belonging for families within their communities.
• Provide information, advice and signposting to families and professionals, supporting appropriate referrals into EACH services.
• Contribute to the development of community-based programmes aligned to EACHs care strategy.
• Work with the line manager to develop and implement a local delivery plan, based on organisational priorities and emerging insight from communities.
• Work collaboratively with clinical, care, volunteering and fundraising teams to ensure a coordinated approach to community engagement.
• Support, involve and coordinate volunteers within community engagement initiatives, recognising their contribution to EACH.
• Support the development of a compassionate communities approach, recognising the role of communities in supporting families beyond hospice services.
• Maintain accurate and up-to-date records of engagement activity, contacts and outcomes in line with EACH policies and systems.
• Coordinate information relating to community engagement activity, partners and volunteers.
• Contribute to data collection, reporting and evaluation of community engagement activity to demonstrate impact, generate insight and inform service development.
• Represent EACH at community meetings, forums and events, promoting the organisation and its values.
• Maintain awareness of the diversity and cultural needs of the communities served by EACH.
• Travel across the region to deliver engagement activities and build community relationships.
• Participate in team meetings, supervision, training and organisational activities.
• Maintain appropriate professional boundaries at all times.
• Take responsibility for continuing professional development, attending training, supervision and appraisal as required.
• Work flexibly to meet the needs of the service, including occasional evenings and weekends.
