Job summary

For entry to the Speciality Doctor grade, applicants must have at least four years of post-registration training, of which at least two years must be in a speciality training programme in Emergency Medicine (for example, ACCS) or must be able to demonstrate comparable skills and competencies.

The job title of the post is Speciality Doctor in Emergency Medicine. The post is paid on the Speciality Doctor scale. Applicants wishing to work part-time would be considered.

Job Plans are worked around a twenty four person rota. The rota provides 10 PA in total with a split of 9 DCC and 1 SPA.

The post holder will be encouraged to advance their career in Emergency Medicine and will be supported in obtaining postgraduate qualifications. The department also has experience of supporting doctors in CESR (previously Article 14) applications, and again this would be encouraged for suitable experienced doctors.

Duties of the Post

This is a CESR training post and as such the post holder will get a dedicated educational supervisor to take them through the CESR programme to ensure submission for successful CESR application. Secondments will be provided in the following specialties: Paediatrics, Acute Medicine, Anaesthetics and Intensive Care Medicine. There will be an annual ARCP conducted in house to evaluate progress and set goals for the next twelve months.

Main duties of the job

The postholder will be expected to undertake full clinical duties including the running of clinics or assessing and managing acutely unwell patients, along with patients presenting with injuries both major and minor. The care may very occasionally include initial resuscitation where appropriate. The postholder will be expected to manage the caseload within urgent treatment and organise immediate management and plan for discharge or where required referral for follow up. The referral could be to GPs to an appropriate specialist team for continued hospital care or to other services.

The job is 10 PA (typically 1.5 SPA 8.5 DCC)The post holder will be encouraged to advance their career in Emergency Medicine and will be supported inobtaining postgraduate qualifications. The department also has experience of supporting doctors in CESR(previously Article 14) applications, and again this would be encouraged for suitable experienced doctors.

Duties of the Post

The postholder is expected to work on the middle grade tier rota in the department and provide the relevantexpertise and experience necessary to ensure the efficient primary management of emergency cases. Thepostholder will be required to work with the other team members in the management of the departmentworkload. The postholder will also be expected to provide supervision to the junior members of the team.The postholder will be expected to undertake full clinical duties including the immediate care of critically illpatients attending the Emergency Department and also patients presenting with injuries both major andminor. The care will include initial resuscitation where appropriate, diagnosis, management and thetreatment, referral and discharge under the guidance of the Consultants. The referral could be to GPs to anappropriate specialist team for continued hospital care or to other services.

The postholder will be expected to follow the various guidelines and protocols issued in the department. Inthe case of major trauma cases, cardiac arrests and critically ill patients attending the department, thepostholder will be expected to act in a team leadership role, following appropriate life support guidelines.

Postholders would be expected to gain and maintain certification in ALS, ATLS etc and APLS or recognisedequivalents to these courses.In light of the changing work practices, the work programme may need to be varied in the future. This willbe done only after consultation with the incumbent of the post. As duty hours require the incumbent beingpresent in the department, no on-call or residential accommodation is offered with this post.In exceptional circumstances, additional duties outside normal rostered hours may be required to ensurethe continuation of essential patient care. So far as is consistent with the proper discharge of the aboveduties, the postholder may be required to deputise from time to time for absent colleagues.An educational mentor will be provided process to make sure that the professional and personaldevelopment of the postholder are maintained. The Consultants will act as educational mentor and willmeet with the postholder on a regular basis.There is the opportunity to undertake CESR training with the department and as such the post holder willget a dedicated mentor to take them through the CESR programme to ensure submission for successfulCESR application. The mentor will be an experienced CES assessor. Secondments will be provided in the8 following specialties: Paediatrics, Acute Medicine, Anaesthetics and Intensive Care Medicine. There will bean annual ARCP conducted in house to evaluate progress and set goals for the next twelve months.

Teaching, Audit & Research

The postholder will be expected to participate in teaching and training of junior doctors, medical students,nurses and paramedical staff as directed by the Clinical Lead. The postholder will also be expected toparticipate in the educational programme and attend relevant postgraduate courses in accordance with thelocal and national CME requirements.The postholder will also be expected to participate in the departmental audit programme. Clinical researchwill be encouraged.

Emergencies and Unforeseen Circumstances

The postholder accepts that they will also perform duties in occasional emergencies and unforeseencircumstances, without additional remuneration at the request of the Clinical Lead or the on-call Consultant.It has been agreed between the professions and the Department that while the postholder accepts that theywill perform such duties, it is stressed that additional commitments arising under this heading areexceptional and the postholder should not be required to undertake work of this kind for prolonged period oron a regular basis.It is the responsibility of the Clinical Lead to determine whether a particular situation can be described asan emergency.

Study and Training

Postgraduate study is encouraged. In-service training is available and clinical meetings are held in theDepartment. There is an excellent postgraduate medical centre at Royal Blackburn Hospital whichprovides excellent library facilities and an active teaching programme, including guest speakers, clinicalconferences and departmental tutorials. Study leave is awarded at the discretion of the Clinical Lead.

The Department

The Emergency Department is located at the Royal Blackburn Teaching Hospital a newly built PFI hospitalwith fully integrated Paediatric and adult areas with Resus, PACS system, and fully supported by CCU andITU Assessments Units, X-ray, CT and MRI facilities. There is a hospital trauma team and cardiac arrest ismanaged in house.In additional to the main Emergency Department which takes primarily ambulance cases, there are 2Urgent Treatment Centres catering to minor injuries and illnesses assessing both walk-in and appointedpatients. One is situated in Burnley General Hospital and other co-located within the Emergency Department at Royal Blackburn Teaching Hospital.The Emergency Department has an 8 bedded resuscitation area (with 1 bed designated for paediatricpatients) with 42 major cubicles. The two Urgent Treatment Centres have examination cubicles, staff base,separate adult and children waiting areas, reception and all facilities to see minor injury and illness. Thedepartment receives excellent support from the Intensive Care Unit, Medical Assessment Unit and SurgicalAssessment Unit, Paediatrics, Orthopaedics, ENT and Maxillofacial Surgery. Serious burns andneurosurgical patients are transferred to the regional centre at Preston or Manchester.The Departments have annual attendances of approximately 170,000 across the three sites.The Acute Medical Unit has been established both for medical patients referred from the EmergencyDepartment and Urgent Treatment Centre and for direct GP referrals. Patients who are referred by theirGP to other specialties are ordinarily directed straight to the appropriate ward or assessment area. Thereare ambulatory medical and surgical day emergency care units.There are currently 18 Consultants and two Specialists who work on the Consultant Rota in post in the unit.1. We also have AdvancedCare Practitioners, Emergency Nurse Practitioners, Physicians Associates and First ContactPhysiotherapists. We train staff of all grades and have medical students of various years from UCLAN andLancaster University in the department.

Terms and Conditions of Service

This post is subject to the Terms and Conditions of Service for Specialty Doctors England 2021. The appointee will be required to live within 10 miles or 30 minutes travel time by car from the RoyalBlackburn Hospital. The Trust will reimburse certain expenses relating to removal and relocation whenrequired, the expenses refunded shall reflect actual costs involved but will not exceed £8,000. If relocationis required this must be discussed as interview.

As well as the nursing team. The department also supports the education of medical students from UCLAN and Lancaster Universities. We are a teaching unit providing teaching for all grades of staff.

You will be assigned an educational supervisor who will meet with you regularly to ensure your educational needs are met whilst you are with us and we encourage Junior Clinical Fellows to undertake the Life Support Courses whilst you are with us. To aid this you will have access to a study leave budget. There is simulation training which is run on the shop floor and that you will be encouraged to attend.
