The Children's Community Learning Disability team in South Staffordshire are expanding and havean exciting opportunity for a creative and innovative Specialist Nurse who has experience of working with children who have a learning disability and their families. In the role you will have the opportunity to work alongside an experienced group of Learning Disability Nurses, Psychologists, Occupational Therapist, SLT and Psychiatrist.

In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs using an inclusive, person-centred approach. Our model places our young people's needs at the heart of what we offer to ensure that they are getting the best care possible. The Children's Learning Disability team offers: support and intervention for young people with emotional and physical healthcare needs who may be at risk of placement breakdown or hospital admission.

You will be responsible for the management of a complex caseload, which

includes, specialist assessment (within service specification), care planning in

conjunction with service user and carer, evaluation of individual treatment

plans for CYP with learning disabilities.

Main duties of the job

• Be responsible for the assessment of mental health and/or medical risk factors relating to children and young people, including co-morbidity (e.g. suicidality, self-harming behaviours and medical risk associated with mental health issues)
• To undertake physical health monitoring as appropriate i.e height, weight, blood pressure etc and be responsible for identifying appropriate action to be taken
• Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
• Complete and contribute robust and dynamic risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve and offer optimal quality of life and health outcomes
• Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
• Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Assist in ensuring that the aims and objectives of the service are fulfilled and to identify factors which may inhibit these from being achieved.
• Required to work within the NMC Code of Practice and professional guidelines
