Looking for a role where you can make a real difference every day?Lordshill Health Centre is seeking a friendly, organised and proactive Patient Navigator to join our established practice team.
Main duties of the job
Key Responsibilities
- Responding to patient enquiries by telephone, online and in person.
- Booking and managing appointments.
- Guiding patients to the most appropriate clinician, service or support pathway.
- Updating and maintaining patient records accurately.
- Working closely with clinical and administrative colleagues to provide an excellent patient experience.
- Supporting the smooth day-to-day operation of the practice.

About the Role
As a Patient Navigator, you will be one of the first points of contact for patients contacting the practice. Whether speaking to patients on the telephone, responding to online requests, or assisting at reception, you will help ensure patients are directed to the most appropriate service efficiently and compassionately.
The role requires someone who can listen carefully, communicate clearly, and remain calm when managing a varied workload.

Previous experience within primary care, healthcare, customer service or administration would be advantageous, although training will be provided for the right candidate.
