<p><strong>Make a Real Difference During the Moments That Matter Most</strong></p><p><strong>Location:</strong> Swindon Intermediate Care Centre<br><strong>Hours:</strong> Night Shifts (8:00pm - 8:00am)</p><p>Join our friendly and dedicated <strong>Out-of-Hours Community Health Navigation Team</strong>, where every shift brings new challenges and opportunities to make a real difference.</p><p>As a <strong>Community Health Navigator</strong>, you will be the first point of contact for patients, healthcare professionals, visitors, and partner organisations, providing a professional, <span class="single_desc_int_gb_word">compassionate</span>, and efficient service.
