This is an exciting opportunity to join a dynamic, friendly and supportive Community Social Work team and support the team to complete assessments in line with the Care Act 2014 and arrange appropriate support.

The post is currently 37 hours but there may be some flexibility either way regarding total hours. We are working in a hybrid way and there will be requirements to work from the office and from home.

The successful candidate will need to evidence that they have experience in understanding and supporting the needs of adults with physical disabilities, learning disabilities and older people.
