We are seeking a motivated and compassionate Assistant Practitioner to join our dynamic Stroke Rehabilitation team at Haywood Hospital for a 12-month secondment, covering maternity leave. Working within our bed-based stroke rehabilitation service, supporting adults following stroke to regain independence and achieve their rehabilitation goals. Main duties of the job

The Assistant Practitioner provides rehabilitation assessment and treatment for patients following stroke, supporting recovery, discharge planning and independence. The role involves managing a delegated caseload, implementing therapy programmes, working closely with multidisciplinary teams, maintaining clinical records, and contributing to service improvement and seven-day service delivery.

Job description

We are looking for an enthusiastic and motivated Assistant Practitioner to join our Stroke Rehabilitation Team. The successful candidate will work closely with therapists and the wider multidisciplinary team to support patients to maximise their independence and achieve their rehabilitation goals.

Key duties include:

• Delivering rehabilitation programmes for patients following stroke and other neurological conditions.
• Managing a delegated caseload and monitoring patient progress within agreed competencies.
• Supporting patients and families with rehabilitation, discharge planning and self-management strategies.
• Working collaboratively with therapists, nursing colleagues and community services to ensure seamless patient care.
• Maintaining accurate clinical records and contributing to high standards of care and governance.
• Supporting service development, quality improvement initiatives and the delivery of a seven-day rehabilitation service.
