Job summary

The Senior Health Care Support Worker plays a vital role within the multidisciplinary Intestinal Failure and Rehabilitation team. Under the guidance and supervision of the specialist nutrition nursing team the post holder will be responsible for providing services in accordance with the Trusts values and behaviours in the delivery of safe and effective patient care for patients with Intestinal Failure.

Main duties of the job

Successfully manage the Intestinal Failure Database and Registry, maintaining accurate patient records and supporting service reporting requirements.

Coordinate Nutrition Nurse activities and clinic schedules to ensure efficient service delivery.

Facilitate patient pathways by creating and managing encounters for the Nurse-Led Rapid Access Clinic.

Ensure timely provision of ancillary supplies and equipment for patients receiving IV therapies.

Lead the administration and follow-up of the Nutrition Nurse Advice Line, ensuring all calls are documented, actioned, and resolved appropriately.
