Job summary

You will be part of a multi-disciplinary team, delivering person-centred substance use interventions and clinical procedures.

• Complete physical and mental health assessments for people who access our services.
• Support in the delivery of pharmacological and psychosocial interventions including assessments.
• Conduct care planning and review of service users treatment.
• Conduct assessments of physical dependency and determine safe introduction/titration to opiate substitute treatments as required.
• Conduct detox assessments.

Job description

• Undertaking physical and mental health assessments for people who access our services when indicating by clinical need, as well as ensuring appropriate sign posting/referrals to primary care or appropriate secondary healthcare services for identified health issues.
• Delivering specific clinical procedures and tasks in line with level of training and competence.
• Supporting in the delivery of pharmacological and psychosocial interventions including assessments, care planning and review of service users treatment.
• Managing/supporting a caseload of service users with varying degrees of complexity.
• Conducting assessments of physical dependency and determining safe induction/titration to opiate substitute treatments as required.
• Conducting detox assessments-identifying the need for specialist medical referrals and the suitability for community detoxification. Where appropriate, making referrals for specialist inpatient services and liaising with hospital and inpatient unit staff.
• Supporting in analysis of incident trends and in service user mortality review processes.
• Completing clinical audits and supporting with medicine optimisation and cost effectiveness.
• Ensuring that there is good communication of treatments offered, treatment progress and outcome of any investigations to offer healthcare agencies, partner agencies and GPs.
• Effective and collaborative communication with colleagues and management.
• Retaining all required competencies and revalidation.
• Attending and actively participating in meetings appropriate to the development and maintenance of the role.
• Ensuring that procedures are constantly reviewed to promote best practice and evidence-based practice in accordance with UK Guidelines on Clinical Management, NICE guidance and Middlesbrough Council protocols and procedures
• Ensuring a high standard of case notes and that all paperwork and recording is undertaken in accordance with Middlesbrough Council Policy and Procedure.
• Ensuring health and safety standards are maintained.
• Maintaining NMC nursing standards and administering medication safely in line with PGD.Ensuring health and safety standards are maintained.
