Job summary

The Jubilee Street Practice Ltd is looking for an organised, proactive and patient-focused Care Coordinator to join our team. The job is being advertised at 22 hours per week. Working arrangements can be agreed with the successful candidate and in line with Working Time Regulations.

This is a key role in helping patients receive the right care at the right time. Much of good general practice happens before and after the consultation: identifying who needs review, contacting patients, helping them understand why they are being recalled, coordinating appointments, tracking follow-up and making sure actions do not get missed.

You will support the practice with proactive recall, planned care, long-term condition reviews, NIS, LES, QOF and local contractual work. You will work closely with GPs, nurses, HCAs, pharmacists, social prescribers, Patient Assistants, administrative colleagues and external services to ensure patient follow-up is clear, safe and timely.

This role would suit someone who is highly organised, confident using digital systems, comfortable speaking with patients, and able to work independently while staying closely connected to the wider team.

Main duties of the job

The Care Coordinator will support the practice in delivering proactive, coordinated and well-tracked patient care.

The role will focus on recall activity, planned care coordination, patient engagement, NIS, LES, QOF and local contractual work, EMIS searches, trackers, reporting systems, external service links and safe escalation of concerns.

The post-holder will work within a non-clinical role, supporting patients to access appropriate care and ensuring clinical questions, safeguarding concerns or risks are escalated to the appropriate clinician or manager.
