Job summary

The post holder will work closely with the ICC Clinical (GP) Lead. Together you will be jointly responsible for the development and ongoing success of a defined Integrated Care Community.

You will be at the forefront of the evolution of integrated neighbourhood working in line with the NHS 10 Year plan and the National Neighbourhood Health Implementation Programme (NNHIP). This is a rare and exciting leadership opportunity to shape the future of truly integrated, community-led care in South Cumbria -- transforming health and social care at neighbourhood level by embedding a unique, innovative model within the NHS.

You will lead the development of a dynamic, multi-agency Integrated Care Community, uniting NHS teams, local councils, VCSFE organisations, primary care, and community assets to deliver population health management, reduce inequalities, and drive prevention-focused innovation.

Experience:

• Experience in service development, transformation or project management in health, care or community settings.
• Strong communication, facilitation and stakeholder engagement skills.
• Ability to work collaboratively across organisational boundaries.
• Confidence using data and evidence to inform decisions.
• A proactive, flexible approach and a passion for community-based care.

Main duties of the job

Key responsibilities include:

Championing the development of the ICC so it fully reflects national priorities around population health, prevention, self-care, and the National Health Framework

Building effective cross-ICC collaboration and robust infrastructure across the Morecambe Bay footprint, local councils, and the wider Lancashire & South Cumbria system

Working hand-in-hand with specialist services to create seamless, integrated pathways for local citizens

Deputising for senior colleagues and providing clear, influential reports on progress, performance, and opportunities

Leading service reviews and supporting skill-mix planning, recruitment, and development to enable new ways of working

Providing strong, visible leadership and line management for the case management team (incorporating Nurses, Allied Health Professionals, Advanced Practitioners and Administrators)

Working collaboratively with the named ICC Clinical (GP) Lead

Co-ordinating modernisation, identifying innovative solutions, and evaluating short cycles of change that make the very best use of local assets

Support co-production of projects / events with citizens and community organisations

About us

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
