About the Role

We are seeking an experienced and motivated Patient Safety Incident Investigator to join our Goodall Division. This is an exciting opportunity to play a pivotal role in improving patient safety, driving learning, and shaping high-quality care across a diverse range of services.

You will lead systems-based, high-quality investigations under the Patient Safety Incident Response Framework (PSIRF), ensuring that learning--not blame--drives improvement. As stated in the JD, the postholder "ensures that investigations are timely, compassionate, methodologically robust, and focused on learning and improvement rather than blame."

What You'll Be Doing

• Leading and coordinating PSIRF-aligned patient safety investigations across the Division.
• Applying systems-based methodologies such as SEIPS, Human Factors, and Just Culture.
• Conducting interviews, evidence reviews, timelines, and systems analysis.
• Producing high-quality investigation reports with clear findings and proportionate recommendations.
• Ensuring compassionate, transparent engagement with patients, families, and carers.
• Supporting staff involved in incidents and promoting psychological safety.
• Presenting findings at governance meetings and contributing to Trust-wide learning.
• Working with clinical and operational teams to embed sustainable safety improvements.
• Providing training and coaching on investigation methods and systems thinking.
• Supporting wider Quality Governance activity when required, including tracking actions and identifying themes.
• Deputising for the Head of Quality Governance when needed.
