Job summary

We have an exciting opportunity for a Specialist Physiotherapist at Buckinghamshire Healthcare NHS Trust to join the Rapid Response & Intermediate Care (RRIC) Teams across Thame and Buckingham. In this community-based role, you will be responsible for supporting patients with complex health needs through personalised rehabilitation and case management approaches. You will work closely with Advanced Community Practitioners and multidisciplinary colleagues to ensure timely, effective care delivery, while supporting workload coordination across the service. You will utilise your specialist clinical expertise to assess patient needs, deliver evidence-based interventions, and help identify individuals at risk of hospital admission, ensuring they receive appropriate support to achieve the best possible health outcomes.

Main duties of the job

As a Specialist Physiotherapist, you will be responsible for contributing to the development and implementation of clinical pathways, ensuring service delivery is aligned with agreed standards, protocols, and evidence-based practice. You will support the creation and ongoing review of operational procedures and guidelines within the RRIC service to enhance consistency and quality of care delivery. You will be responsible for supporting service resilience by participating in 7-day working rotas, including weekend cover and Urgent Community Response duties across Buckinghamshire localities. In addition, you will be responsible for supporting service improvement initiatives by identifying opportunities to enhance efficiency, safety, and effectiveness within community-based rehabilitation services.

Job description

  • To ensure accurate and timely maintenance of electronic patient records, updating clinical activity in line with Trust information governance standards and ensuring documentation is complete, secure, and audit-ready at all times.
  • To support the coordination of staff activity by contributing to the allocation, monitoring, and oversight of delegated clinical tasks, ensuring effective use of workforce resources within agreed service frameworks.
  • To maintain clinical competence through ongoing professional development, engaging in advanced training, policy review activities, and contributing to the evolution of professional standards within the service.
  • To support the implementation of Trust policies and national guidelines by ensuring adherence to operational procedures, regulatory requirements, and best practice frameworks across all areas of service delivery.
  • To contribute to the development and delivery of training, supervision, and mentorship for junior staff and learners, supporting skill development and promoting consistent standards of practice across the team.
  • To promote effective communication across internal and external stakeholders by ensuring timely sharing of relevant service information, supporting continuity of operational activity and service responsiveness.
