Job summary

We have an exciting opportunity for a CAMHS Senior Practitioner at Herefordshire and Worcestershire Health Care NHS Trust to join the Child and Adolescent Mental Health Service at Worcestershire Royal Hospital. You will be responsible for delivering high-quality, community-based mental health care to children, young people, and their families as part of a multidisciplinary team. You will support the assessment, planning, and delivery of person-centred care while promoting positive outcomes for service users. You will be expected to act as a professional role model, uphold the Trust's values, use your initiative, and take responsibility for maintaining high standards of clinical practice and service delivery. As part of a developing service, you will also have the opportunity to contribute to service transformation and work across different teams as required.

Main duties of the job

As a CAMHS Senior Practitioner, you will be responsible for managing a defined caseload of children and young people aged 018, coordinating care plans, monitoring clinical progress, and ensuring appropriate interventions are delivered throughout their care journey. You will liaise with a range of external agencies and specialist services, including Tier 4 inpatient units, the Mental Health Liaison Team, and the Home Treatment Team, to facilitate effective care pathways and continuity of support. You will also contribute to the management of referrals from a variety of professionals and organisations, ensuring timely access to services and maintaining accurate clinical documentation in line with Trust policies.

Responsibilities of this job role

To complete comprehensive specialist assessments, including risk assessments and personalised care plans, ensuring these are regularly reviewed and developed in partnership with patients and, where appropriate, their families to support effective clinical outcomes.

To deliver evidence-based therapeutic interventions through individual, family, and group sessions, while offering specialist advice and clinical guidance to healthcare professionals involved in patient care.

To act as the key worker for an allocated caseload, coordinating patient pathways, monitoring progress, and ensuring continuity of care through effective liaison with all relevant stakeholders.

To produce accurate and timely clinical documentation, including assessments, treatment reports, progress records, and discharge summaries, ensuring compliance with local policies, professional standards, and statutory requirements.

To provide clinical services across a range of settings, including hospitals, community facilities, schools, residential homes, and patients' homes, adapting interventions to meet individual needs and circumstances.

To participate in clinical supervision, safeguarding processes, referral triage, and specialist practice initiatives, while promoting mental health awareness and maintaining strong partnerships with healthcare, education, and community organisations.
